, Inleiding.
Mijn naam is Manuela Tuik, ik ben 23 jaar oud en momenteel werkzaam als leerling mbo-
niveau 4 verpleegkundige bij Beter Thuis Wonen thuiszorg in Hoogezand. Ik zit in mijn
laatste jaar van de verpleegkundige opleiding.
Voor deze opdracht ga ik mij verdiepen in de evaluatie momenten binnen onze instelling met
de zorgvrager, hierbij wordt benoemd op welke wijze de gegevens worden geregistreerd,
welke formulieren hierbij gebruikt worden. En op welke wijze er wordt ingegaan op de
ervaringen, ideeën, behoeften en gevoelens van de zorgvrager.
