Inspectie: zwelling, verkleuring, temperatuur

ROM art. glenohumeraal
AROM en PROM: anteflexie, retroflexie, exorotatie, endorotatie, abductie, adductie

Specifieke testen kapsulaire mobiliteit
Hyperabduction test
- Patient in zit
- Hand 1  fixatie clavicula en scapula zodat er geen elevatie is
- Hand 2  bij elleboog
- Vervolgens wordt er passieve abductie uitgevoerd
- Interpretatie: test is positief als er 105° of meer abductie
uitgevoerd kan worden. Dit duidt op laxiteit van het inferior glenohumeraal ligament
(kapsel).

Horizontale retroflexie test
- Patiënt in zit
- Hand 1  fixatie laterale rand van scapula, net onder de oksel
- Hand 2  onder elleboog
- Vervolgens arm in maximale abductiestand en vanaf hier naar retroflexie bewegen. Opletten
dat arm in scapulaire vlak staat als 0-stand.
- Interpretative: test is positief wanneer er naar meer dan >50 graden retroflexie bewogen
kan worden. Dit wijst op vergrote beweeglijkheid van het voorste kapsel.

Horizontale adductie test
- Patient in zit. Therapeut staat aan niet aangedane zijde.
- Hand 1  fixeert scapula
- Hand 2  onder elleboog, onderarm patiënt steunt op
onderarm van therapeut.
- Vervolgens beweegt therapeut de arm van de patiënt naar
adductie terwijl de scapula gefixeerd wordt.
- Interpretatie: test is positief wanneer er naar meer dan >90 graden adductie bewogen kan
worden. Dit wijst op vergrote beweeglijkheid van het achterste kapsel.

, Specifieke testen cuff ruptuur letsel
Lift off test
- Patiënt in stand met hand op lumbale gedeelte van de rug.
- Vervolgens patiënt instructie geven om de hand van de rug af
te bewegen.
- Interpretatie: test is positief wanneer patiënt de hand niet
van de rug af kan bewegen.

Bear hug test
- Handpalm met extensie van vingers van aangedane schouder op
andere schouder leggen.
- Vervolgens moet patiënt deze positie handhaven terwijl
therapeut d.m.v. exorotatie probeert weerstand biedt.
- Interpretatie: test is positief als de patiënt de positie niet kan
handhaven of wanneer de herkenbare pijn optreedt.
- Subscapularis

Drop sign
- Patient in zit. Arm in 90° abductie en elleboog 90° flexie. Zorg voor
maximale exorotatie (ondersteunen met beide handen)
- Vervolgens haalt therapeut zijn hand om de pols van patiënt weg. Dit
doen in 2 posities, de 1 met meer abductie dan de ander.
- Interpretatie: test is positief als patiënt de positie niet kan handhaven

Lag sign
- Patient staat met hand van pijnlijke schouder op de lumbale
regio (achter de rug).
- Therapeut beweegt met 1 hand de pols van de patiënt van de
rug af en andere hand houdt elleboog vast.
- Vervolgens laat therapeut de pols los en moet patiënt deze
positie actief handhaven.
- Interpretatie: test is positief voor scheur in pees van
subscapularis wanneer patiënt de pols niet actief op de plek kan houden wanneer therapeut
de pols loslaat.

Full can
- Patient staat met armen in 90° anteflexie en lichte abductie.
- Vervolgens moet patiënt deze positie handhaven terwijl
therapeut van bovenaf weerstand geeft.
- Interpretatie: als empty can test positief is en de full can
negatief, komen de impingement klachten niet door rotator cuff
letsel.

Empty can
- Patient in stand of zit. Armen in 90° anteflexie, lichte
abductie en volledige endorotatie (duimen naar de grond)
- Vervolgens moet patiënt deze positie handhaven terwijl
therapeut weerstand vanaf bovenaf geeft.
- Interpretatie: test is positief wanneer er zwakte of pijn is in
aangedane arm t.o.v. niet-aangedane arm.
