Pijnmechanismen en psychosociale factoren1. Stressor
- Er is een stressor en reageert met een pijnervaring.
Tussenin gebeuren pijnmechanismen.
Pijnervaring bestaat uit 3 delen:
o Sensoriële dimensie: waar voel ik het?
o Emotionele dimensie: het gevoel erbij
o Cognitieve dimensie: je gaat er een betekenis aan geven.
Die pijnervaring zorgt voor gedrag aanpassingen en fysiologische acties veranderen.
2. Soorten pijn
Nociceptieve pijn - Afkomstig van nociceptieve prikkeling
Nociceptoren liggen in het weefsel.
3 groepen en kan hiertussen fluctueren/overgaan:
o Inflammatoir: je hebt weefselschade en het proces van
weefselherstel zorgt voor een inflammatoire reactie.
Vb. scheuren van een ligament.
Via een klinisch patroon (niet vanuit wetenschap).
Perifere sensitisatie: de receptoren worden gevoeliger
voor pijnprikkels.
De hersenen zullen dan meer receptoren maken en dit
is dan centrale sensitisatie.
o Mechanisch: hier is geen weefselschade.
Vb. tegen een deur lopen.
Je hebt een hoog stimulus nodig.
Hierdoor krijg je
o Ischemisch: hier is geen weefselschade, maar er is een
tekort aan zuurstof.
Langdurige stimulus zorgt voor verminderde circulatie
en weefselvocht.
Dit zorgt voor een prikkeling zodat je je verplaatst.
Neuropathische pijn - Beschadiging zenuwstelsel
Perifere letstels leidt tot verandering bij alles erboven.
Dit veroorzaakt spontane activiteit.
o Dit kan continu, burst of geen activiteit zijn.
o Je krijgt andere ionkanalen en het maakt je gevoelig voor
andere substanties:
Mechanosensitief: rek verhoogd de pijn.
Bij druk direct pijn.
Ischemosensitief: verminderde O2 verhoofd de pijn.
Bij druk even aanhouden en dan pijn.
Chemosensitief: ontsteking verhoogd de pijn.
