Circumstances of Referral:

Jane Doe is 22 years old female who was brought to emergency after she overdosed on

her prescription medicines. After spending 36 hours in intensive care unit she was referred to

adult psychiatry wing. Apart from suicide attempt, signs of self-mutilation on legs and arms were

the reason for referral. According to her the cause of her suicide attempt was breaking up with

her boyfriend and she claims that she didn’t have any suicidal intentions, instead she wanted a

break from what was happening and overdosed in attempt to sleep for a long time. Patient has a

history of depression, anxiety and panic attacks. She is currently not attending her college and is

working at a small café as waitress, that’s where she met her boyfriend. Jane talks about feeling

sad and lonely along with feeling of emptiness. She also feels she is a failure with her not

attending college and working at café just so she can barely support herself. She wants to move

out of her parents’ house but can’t due to financial reasons. She also keeps her mental health

issues from her parents resulting in inability to properly get treatments as she is on her family

medical plan.
