De verdeling van water in het menselijk lichaam op kwantitatief niveau
De totale hoeveelheid vocht in het menselijk lichaam (Total Body Water) is voor mannen
60% en voor vrouwen 50% van het totale lichaamsgewicht. Van dit TBW is 60% intracellulair
vloeistof (ICF) en 40% extracellulair (ECF). Extracellulair vloeistof kan op zijn beurt weer
worden onderverdeeld in interstitieel vloeistof (75%), bloedplasma (20%) en transcellulaire
vloeistof (5%). Hierboven staan de getallen weergegeven voor een gemiddelde man van 70
kg.
Wat opvalt in de figuur is dat hoewel de Osmolenconcentratie (vrije deeltjes in een
oplossing) varieert binnen de verschillende compartimenten, de osmolariteit overal
hetzelfde is nl: 290 mOsm.

De kwantitatief en kwalitatief belangrijkste verschillen tussen het intra en extracellulaire
compartiment
Kwantitatief zijn de verschillen tussen intracellulair en extracellulair dat en intracellulair een
veel lagere Na+ concentratie in de ICF maar wel een veel hogere K+ concentratie in het ICF in
vergelijking met het ECF. Tot slot is er kwantitatief nog een veel hogere Cl- concentratie in de
ECF in vergelijking met het ICF.
Kwalitatief betekent dit dat de Na+ en Cl- stroom van extracellulair naar intracellulair gaat en
dat de K+ stroom van intracellulair naar extracellulair gaat.
Wat ook handig is om te begrijpen is wat er gebeurt als je bijvoorbeeld een infuus aanlegt.
Als het infuus een fysiologische zoutoplossing bevat dan zal alleen het compartiment waar
het infuus is aangebracht (ECF) opzwellen en groter worden met de hoeveelheid die in het
infuus zat. Als je echter i.p.v. een zoutoplossing puur water geeft dan zal de osmotische
waarden van de compartimenten veranderen. Ten eerste zal het compartiment waar de het
water in ingebracht opzwellen met hetzelfde volume als in het infuus zat. Daarna zal de
vloeistof zich zo verdelen dat er een evenwicht ontstaat tussen de compartimenten. Als je in
tegenstelling tot hiervoor alleen zout toevoegt zal in het begin de osmolariteit in het
compartiment waar het zout is toegevoegd (ECF) veranderen (meer mOsm). Daarna zal er
water vanuit het ICF naar het ECF stromen om een evenwicht te vormen in osmolariteit.
