In de voorgeschiedenis is een verband tussen:
● Ontstaan van de aandoening
● Psychosociale stressfactoren, emoties en / of psychische conflicten
Kan niet verklaard worden door een organische afwijking. Een combinatie met een
organische - of habituele stemstoornis is wel mogelijk.
Larynxbeeld
(Vaak) ontbreekt een organische afwijking; de stemplooien en arythenoïden hebben een
normale structuur en mobiliteit tijdens bijvoorbeeld ademhalen / hoesten / lachen.
Sterke variatie in mobiliteit larynx; hyper- ←→ hypokinetisch
→ Altijd KNO-onderzoek doen om organische of neurologische pathologie uit te sluiten,
differentiaaldiagnostiek.
De cliënt is zich er niet van bewust dat de mechanismen die gebruikt worden voor
bijvoorbeeld kuchen, schrapen en lachen dezelfde zijn als die voor stemgeving.
,Differentiaaldiagnostiek
Drost (2009) onderscheidt psychogene stemstoornissen van:
● Organische stemstoornissen; KNO-onderzoek
● Habituele stoornissen; veroorzaakt door overbelasting spreekstem, vaak gerelateerd
aan spreekberoepen
● Mutatiestoornissen; gerelateerd aan leeftijd, geslacht en puberteit. Na de puberteit
geen normale ontwikkeling van de mannen- of vrouwenstem.
De Bodt (2015) onderscheidt psychogene stemstoornissen van:

● Counseling, therapeut moet enkele counseling principes beheersen
● Bij valse stemplooistem inspiratoir foneren

Therapie opbouw
● Oproepen van gewenste stem via bijvoorbeeld kuchen, lachen, schrapen (onbewust
of reflexmatig)
● Kuch (of…) verlengen met klinkers “uuh”, “ooh”, of “aah”
● (indien nodig eerst kuch en dan) productie van klinkers
● Oefenen nonsyllaben
● Oefenen korte en daarna ook lange woorden (eerst met klinker beginnend)
● Zinnen
● Lezen
● Eenvoudig gesprek
● Gesprek met bekende binnen de therapiesituatie
● Transfer naar en generalisatie van de stem in de dagelijkse situatie
Belangrijk: zeg niet tegen je cliënt “je kunt spreken zoals je zou willen”. Vertel in plaats
daarvan wat er fysiek aan de hand is:
● De werking van de stemplooien is ‘ontregeld’ of ‘ontspoord’
● Je stemplooien staan nu stil in open stand
● Het lukt je op dit moment niet om de stemplooien aan het trillen te krijgen
