Population Health Approach for Multiple Chronic Conditions
Problem of Multiple Chronic Conditions may not be addressed in the Current Disease
Activities
The issue of the multiple chronic situations may not be adequately addressed in the
current disease activities because individuals are not keenly following the proposed living
standards. For instance, increasing obesity makes the matter worse by increasing the chances of
chronic diseases. Also, people are persistently smoking in high number in the adult population,
which contributes to worsening the issue (Parekh et al., 2011). Besides individuals are having
insufficient physical activities which are a threat to their health. These behavioral choices are
leading to an increased incidence of chronic disease. Chronic disease treatment is also extremely
