HOOFDSTUK 3 : ANTI-AGGREGANTIA EN ANTICOAGULANTIA1. Hemostase
Bloedstolling = stoppen van bloedverlies bij schade aan een bloedvat
3 belangrijke stappen:
Vasoconstrictie -> samentrekking van het bloedvat om bloedverlies te beperken
Thrombocytenaggregatie -> vorming bloedprop
bloedplaatjesactivatie:
- hechten zich aan beschadigd bloedvat (= adhesie)
- klitten samen (= aggregatie)
- hechten zich aan fibrinogeen en vormen een netstructuur
Thromboxaan A2 speelt belangrijke rol bij activatie bloedplaatjes
Coagulatie -> vorming stevige bloedklonter
activatie stollingscascade leidt tot omzetten van fibrinogeen naar fibrine (onoplosbaar eiwit)
fibrine + bloedplaatjesprop = netstructuur -> rode bloedcellen en eiwitten in gevangen ->
stevige bloedklonter
2. Thrombose
Bloedklonter = trombus
ongewilde klonter in een bloedvat
2 soorten trombose:
- arteriële trombose (= bloedklonter in een slagader)
- veneuze trombose (= bloedklonter in een ader)
arteriele trombose :
aanvoer van zuurstofrijk bloed is verhinderd
minder frequent, maar meestal ernstigere gevolgen
klonter kan loskomen en op een andere plek in lichaam bloedtoevoer afsluiten = embolie
zo ontstaat een hartinfarct en een herseninfarct (beroerte)

afvoer van zuurstofrijk bloed is verhinderd
bloed blijft « vastzitten » -> paars en opgezwollen
klonter kan loskomen en op een andere plek in lichaam bloedtoevoer afsluiten = embolie
zo ontstaat een longembolie
3. Geneesmiddelen die inwerken op hemostase
a. Anti-aggregantia:
effect op bloedplaatjes
effect op primaire hemostase (= vorming primaire bloedprop)
acetylsalicylzuur : Aspirine®
WERKING
irreversibele remming van enzyme Cyclo-oxygenase (COX)
remming van vorming thromboxaan A2
thromboxaan A2 is nodig voor de activatie en aggregatie van bloedplaatjes
effect houdt lang aan, want enzyme irreversibel geremd, werkingsduur = levensduur van
bloedplaatjes (7-10 dagen)
INDICATIE
secundaire preventie arteriële trombose
- vroeger doorgemaakt hartinfarct
- vroeger doorgemaakt herseninfarct
- na angioplastiek (ballondilatatie)
- angina pectoris
- (- voorkamerfibrillatie)
Acute fase hartinfarct – herseninfarct
300 mg zo snel mogelijk na eerste symptomen (oplosbare vorm!)
BIJWERKINGEN
verhoogde bloedingsneiging
maaglast
bronchospasmen bij astma-patiënten
overgevoeligheidsreacties met huiduitslag
CONTRA-INDICATIES
overgevoeligheid bij astma-patiënten
maagzweer
stollingsstoornissen
hersenbloeding, maag-darmbloeding
ernstige lever – en nierinsufficiëntie
