Amblyopie
Binoculaire interactie is een normaal fenomeen. Het wordt ook wel binoculair enkelzien genoemd.
Fixatie disparatie: één punt, dat bij fixatie in een oog op de fovea afgebeeld wordt en in het andere oog
niet op het corresponderende netvliespuntnetvliespunt, maar wel binnen P.E.K. Er is wel (verminderd)
enkel zichtenkelzicht en fusie van beide beelden.
Foveale suppressie is is aantoonbaar met 4 prsm basis temporaal.
Amblyopie wordt gedefinieerd als een defecte gezichtsscherpte in een of beide ogen, die aanhoudt na
correctie van een pathologisch obstakel voor het gezichtsvermogen.
Er zijn twee mechanismen betrokken bij de ontwikkeling van amblyopie:
1. Vormdeprivatie
Het verlies van vormvisie
Volledig, wanneer geen stimulus de fovea bereikt
Gedeeltelijk, wanneer er een verslechterd beeld is.
Deze factor is het passieve of statische element bij de ontwikkeling van
amblyopie.
Het is de vraag of er sprake is van volledige prikkelgebrek, aangezien er wat licht (of
'witte ruis') in het oog komt, zelfs bij dicht cataract of unilaterale volledige ptosis.
Betrokken bij bilaterale amblyopie.
2. Abnormale binoculaire interactie
Abnormaal binoculair enkelzien
Treedt op wanneer incompatibele afbeeldingen op de fovea worden gevormd.
Betrokken bij alle soorten unilaterale amblyopie, maar op voorwaarde dat elk oog in
dezelfde mate wordt aangetast
Ontstaan van abnormale binoculaire interactie:
Neuronen van de twee ogen worden verondersteld te strijden om controle over corticale
verbindingen tijdens de ontwikkelingsperiode die van het betere oog krijgen controle ten
koste van neuronen van het aangedane oog.
Deze factor is actief of dynamisch, in tegenstelling tot de statische deprivatiefactor.
Beide factoren zijn betrokken bij alle soorten unilaterale amblyopie.
Von Noorden heeft verklaard dat abnormale binoculaire concurrentie de overhand heeft bij
strabismische amblyopie. Hij suggereerde dat een perifeer object dat op de visuele as van het
