Wat is een pneumonie?
Inflammatoir proces van het longparenchym
Oorzaken infectieus vs niet infectieus
- Infectieus: bacterieel, fungaal of viraal
- Bacteriële pneumonie komen het meeste voor
- Niet-infectieus: chemische oorzaken (aspiratie), radiotherapie, allergische mechanismen of
een organiserende pneumonie
Uitgaande van een infectieuze oorzaak, pneumonie = een acute symptomatische infectie van de
onderste luchtwegen waarbij een nieuwe infiltraat wordt aangetoond.

Continue blootstelling van luchtwegen aan fijn stof of micro-organismen
We hebben wel host defense
Een van die defense is de cillia klaring, omhoog transplanteren van het slijm
Humorale en cellulaire afweer mechanisme zijn er ook
In de longen komen standaard bacteriën voor, een microbioom
Pneumonie is het gevolg van
- Defecte host defense
- Blootstelling hoog virulent micro-organisme
- (te) groot inoculum

- Remming of resistentie fagocytose
Predisponerende host factoren

- Immuun gecompromitteerd
- Metabole factoren (als je ondervoed bent,
vitaminen te kort)
- Blootstellingen (champignon of duiven,
inhalatie van chloor)
- Medische ingrepen (intubatie)
Pathogenese pneumonie: inflammatoire respons
Bacterie komt binnen. Allerlei cellulaire en
humorale immuunprocessen
Influx van cellen en cytokinen. Inflammatie
Wand van de capillairen wordt
toegankelijker. De barrière tussen alveolus
en capillairen wordt minder goed
Als cytokinen ook in het bloed komen kan
het overgaan in een septische pneumonie
, Community-aquired vs nosocomiale pneumonie
Community-aquired pneumonie CAP = acute symptomatische infectie van de lagere
luchtwegen die buiten het ziekenhuis of een verpleeghuid is ontstaan, waarbij op
beeldvormend onderzoek een nieuw infiltraat wordt aangetoond.
Nosocomiale pneumonie/hospital-acquired pneumonie HAP = pneumonie die zich na > 48
uur na opname ontwikkeld.

CAP komt veel voor met hoge morbiditeit en mortaliteit

Patiënten met CAP die opgenomen worden blijven gemiddeld 9 dagen in het ziekenhuis

Koorts
Hoesten (met of zonder sputum)
Dyspneu
Pleurale pijn als de pleura mee doet
In de longen zitten geen gevoelssensoren, maar in de pleura wel. Dit kan behoorlijk pijn
doen.
Tachypneu, verhoogde ademarbeid
Tachycardie
Hypoxemie
Alveoli vullen zich met vocht en cellen. Diffusie is vermoeilijkt. Lucht kan niet goed in de
alveoli. Dan krijg je shunting.
Bij een septisch beeld eventueel hypotensie, eventueel verminderd bewustzijn
Bij LO
Percussie: demping ter plaatse van
consolidatie (en/of pleuravocht)
Auscultatie
- Verminderd ademgeruis bij afgesloten
bronchus
- Bronchiaal ademgeruis bij open bronchus
- Crepitaties en/of rhonchi
Crepitaties is open en dichtvallen van de
alveoli
Rhonchi vaak door slijm, zwaardere
geluiden. Turbulentie van lucht dat langs
slijm beweegt.
De middenkwab kan je alleen in de flank
danwel ventraal beluisteren
