The effectiveness of a picosecond laser in treatment to reduce
facial melasma

Abstract
Problem definition: The picosecondlaser lacks evidence-based confirmation about its
effectiveness in treating melasma in women with Fitzpatrick skin types 3 to 5. Objective: The
aim of this study is to discover whether the use of a picosecond laser is effective in the
treatment of melasma in women with Fitzpatrick skin types 3 to 5. Research question: “Is
treatment with a picosecond laser (532 to 1064 nm) an effective method for reducing facial
melasma in women with Fitzpatrick skin types 3 to 5?” Method: From October 1, 2020 to
December 31, 2020 a systematic search for useful literature was conducted in the Cochrane,
Pubmed, CINAHL and Embase databases. The methodological quality of the studies was
assessed using EBRO assessment forms. Results: This descriptive quantitative literature
review analyzes five publications, including three split-face randomized controlled trials (level
B), a split-face clinical controlled trial (level C) and a split-face pilot study (level C). All
publications reported a decrease in melasma. However, only three studies were found to be
statistically significant. Conclusion: Since all included publications report a decrease in
melasma, it is likely that the picosecondlaser is working. However, due to a lack of high
methodological quality, statistical significance, and clinical relevance, this has not yet been
scientifically proven. While this study provides insight for skin therapists, no practical
recommendation can be given.
