Introduction-
I will be discussing 3 case studies regarding how working practices are
used to successfully meet individual needs. For case study 1 I will be
discussing Alice Fernandez. Alice Fernandez is a 74-year-old woman
who has recently lost her husband due to cancer of the liver, Alice is also
an alcoholic and rather than spending her pension on healthy food, she
is spending it on her drinking habit. Along with this, she has also been
prescribed anti-depressants by her G.P, however, the anti-depressants
are making her lethargic, this could be because of her mixing alcohol
with her the anti-depressants, or they may not be the correct suit for her.
Alice attends an over-65s aerobics class at the centre, however, the
manager of the centre says Alice is causing distress to the trainer and
the other class members due to her frequently smelling of alcohol. The
manager then said she can attend the class if she stands away from
others.
For case study 2 I will be discussing Patrick Green. Patrick Green is a
26-year-old male who lives with his partner James. James had a brief
relationship with someone who was HIV positive, as a result of this,
Patrick is anxious about his health due to him and James not using
condoms. Patrick discusses his worries with his martial arts trainer at the
centre, who then referred Patrick to a sexual health adviser Simon.
Finally, in case study 3 I will be discussing Martin Smithers. Martin
Smithers is a 45-year-old man who has recently been made redundant
from his factory job and is finding it difficult to find a new occupation.
Martin is also finding it increasingly difficult to find the money for
household bills and fears he may be evicted from his apartment. Martin
attends a carpentry class at the centre however, he is finding it difficult to
attend due to the stress of his money issues his chronic skin condition
has become increasingly worse.
, Multi-disciplinary Teams-
A multi-disciplinary team is a group of professionals from different care
disciplines (PowerPoint Moodle, no date), for example, for an individual
suffering from cerebral palsy, their multi-disciplinary team would consist
of a physiotherapist, a G.P, an occupational therapist, a speech
therapist, an advocate and a domiciliary health care assistant. Multi-
disciplinary teams are mostly used for individuals who require complex
needs, such as a person going through cancer treatment. According to
the NHS, “a multidisciplinary approach involves drawing appropriately
from multiple disciplines to explore problems outside of normal
boundaries and reach solutions based on a new understanding of
complex situations”, (MDT - NHS England, January 2015)
Alice would benefit from a multi-disciplinary team due to her requiring
support from a range of professionals, she requires support from her
G.P, in conjunction with a bereavement counsellor to discuss her
antidepressants and to support her through the bereavement of her
husband. Moreover, she requires support from a specialist due to her
liver problems, she could also benefit from support from a social worker
to try to help her living situation. Alice also requires support to help with
her substance abuse, so if possible, a sponsor could support her.
Furthermore, Patrick would benefit from a multi-disciplinary team due to
his complex sexual and emotional health needs. Patrick requires support
from a sexual health professional to discuss how they can both live their
lives while being HIV positive, and also what lifestyle changes they may
need to adapt to. He may also require a counsellor or therapist due to
the possible trust issues or relationship issues that may take place due
to the aftermath of the relationship that James was involved in. They
both will require a G.P to support them and to prescribe what medication
they will be needing to take. Finally, the nutritionist will also be involved
in the multi-disciplinary team.
Moreover, Martin will benefit from a multi-disciplinary team due to his
emotional and physical problems. Martin is finding that his chronic skin
condition is worsening due to the stress he is experiencing as a result of
his financial issues, as well as the worry of him being evicted due to not
being able to afford household bills. As a result of this, Martin would
benefit from a multi-disciplinary team consisting of the G.P.,
dermatologist, to advise Martin on his skin condition and what steps he
