HepatitisDe hepatitis virussen zijn belangrijke virussen die veel voorkomen bij de mens, vooral type A ( HAV,
picornaviridae), type B (HBV, hepadnaviridae) en type C (HCV, flaviviridae). Daarnaast zijn er heel
veel andere types van virussen (niet-hepatitis) die de lever kunnen aantasten (vaak tijdelijk).
Virale hepatitis - Belangrijk
Virale hepatitis kan opgedeeld worden in infectieuze hepatitis en serum hepatitis. Infectieuze
hepatitis omvat hepatitis A en E. De transmissie is faeco-oraal. De serum hepatitis omvat hepatitis
BD en C en de transmissie is parenteraal (buiten het spijsverteringskanaal om; via speeksel, bloed,
seksueel, etc.). Hepatitis B en D komen samen voor. Hepatitis C was belangrijk in het verleden, het
duurt jaren voordat de symptomen komen waardoor het virus goed kan verspreiden. Het
belangrijkste aan hepatitis is dat hepatitis A infectieus is en hepatitis BD serum is. Dan hebben we
nog non-A en non-B hepatitis, dus type E (infectieus) en type C (serum). Ook zijn er nog andere,
minder belangrijke virussen (G, TTV, SEM). Deze virussen (vooral type B en C) tasten de lever aan en
kunnen zeer ernstig zijn (vervetting, cirrose en dramatisch leverkanker).
Hepatitis type A (HAV)
Het HAV behoort tot de picornavirussen. Dit is een grote en belangrijk virus familie waartoe ook de
polio-, coxsackie-, echo-, entero- en rhinovirussen (typisch verkoudheidsvirus) toe behoren. Het
enterotype 72 is HAV. De familie van de picornavirussen is enorm groot en het tast verschillende
organen aan. Het hepatitis virus (moet je kunnen tekenen, zie HBV) tast de lever aan en geeft
hepatitis.
De transmissie van HAV verloopt via eten en drinken. Er kunnen antilichamen gemaakt worden tegen
dit virus (seroprevalentie). Dit is erg hoog wanneer je leeft in bijvoorbeeld Afrika (waar het virus veel
vaker voorkomt), dan in onze regio’s. We zien een acute virale hepatitis met meestal volledige
genezing. De mortaliteit is 0,1% bij kinderen; bij volwassenen ongeveer 1% (vooral ouderen). Een
geïnfecteerd persoon is een paar dagen erg ziek (overgeven, diarree), maar voelt zich erna beter.
Zoals al is gezegd, verloopt de transmissie van faeco-orale overdracht (besmet voedsel, melk,
oesters, drinkwater, etc.). De incidentie is gecorreleerd aan gebrek aan hygiëne. Plotse epidemieën
kunnen ontstaan (bijvoorbeeld in vakantie- en/of legerkampen). Ook is de overdracht parenteraal
mogelijk (bijvoorbeeld door besmette naalden). Op kinderleeftijd gaat de infectie meestal
onopgemerkt voorbij, bij volwassenen is de infectie ernstiger. Preventie van HAV kan gedaan worden
