Impact of the Problem on the Patient

Tracheostomy is a common procedure in the pediatric intensive care unit, with advances

in technology chronically ill children are living longer with the support of a tracheostomy tube.

Immediate postoperative tracheostomy care is a complex and critical time for wound healing.

The airway in children is considered most critical for the first five days post-operatively. The

trachea in children is much smaller in size than in adults, therefore, if decannulation of the trach

were to occur the risk for mortality becomes more critical. Children are considered to have

immature and developing skin, healing rates rapidly differ in each developmental stage. Children

have less muscle mass than adults which may also, contribute to their risk of pressure injuries. In

the pediatric population, the most common reason for pressure injury is due to medical devices.

Pediatric tracheostomies have a complication rate as high as 60%, with 30% attributed to wound

complications in children, and 40% in infants. Tracheostomy wound complications are caused by

infection, granulation tissue formation, skin breakdown, and pressure injuries from securement

devices or from the trach itself.



Impact of the Problem on the Organization

Tracheostomy wound complications impact the organization by extending the recovery

time and length of hospital stay, which increases the cost. Pressure ulcers and other wound

, complications related to the tracheostomy cost the healthcare system approximately 11 billion

dollars annually. This preventable clinical problem causes serious consequences to the patient

and the public. Standardized clinical practice guidelines should be considered to reduce the

impact that wound complications have on the organization.

P – Pediatric postoperative tracheostomy patients, are the patient/population/problem I

have identified for the clinical practice problem.

I – Standardized tracheostomy wound care is the intervention identified for the clinical

practice problem.

C – The comparison is not implementing standardized tracheostomy wound care.

O – The outcome identified for the clinical practice problem is, decreased hospital stay

due to tracheostomy wound complications.

Evidence Based Practice Question

Among pediatric patients at risk, does standardizing tracheostomy wound care reduce the

number of wound complications, compared to not implementing standardized wound care, by

decreasing postoperative hospital length of stay.
