Sharp purser test (graad 4 test)
Sensitief 19-69% & specifiek 71-98%.
Uitvoering:
Patiënt zit rechtop met de nek ontspannen in een semi flexie
positie (dubbelkin). Dit kan al zorgen voor symptomen zoals
compressie. De fysio zet 1 vinger op de C2 van de nek en de
andere hand op het voorhoofd. Duw vervolgens rustig met de
hand tegen het voorhoofd en palpeer met de andere hand de
wervels.
De test is positief als er een vergrootte ROM is, knak geluid of
glij beweging is.
Ligament transversum wordt getest.
Alare ligament shear test (graad 4 test)
Sensitief en specifiek.
Het test de kwaliteit van opzij buigen met je hoofd en rotatie tussen de
Atlas en de axis. Als het positief is dan is er sprake van hypermobiliteit en
een zacht eindgevoel.
Uitvoering:
Patiënt in ruglig. Ondersteun het hoofd en palpeer bij C2. Buig het hoofd
lichtelijk naar de zijkanten en in lichte flexie en kijk naar de ROM en let op het eindgevoel. Doe dit
aan beide kanten. Ook doe je dit met rotatie.
Ligament alaria wordt getest.
Anterior stability test / transverse ligament test (graad 4 test)
Sensitief 65% en specifiek 99%.
Uitvoering:
Patiënt in lig. Kussen onder het hoofd. Pak de occiput met je
handpalmen. Je vingers tussen occiput en C2. Duw met je vingers
het hoofd licht omhoog.
Positief bij symptomen van pijn die niet in dat gebied voor horen te
komen zoals iets in de keel of compressie. Anterior shear test ook
wel genoemd.
Ligament transversum wordt getest.
