Ontwikkeling leeruitkomsten geëvalueerd door student, werkbegeleider en
stagedocent

Tussenevaluatie:
● Voor de tussenevaluatie vult de student het formulier in en geeft zichzelf per rol een
cijfer.
● De werkbegeleider geeft een indicatie van de voortgang van de stage middels feedback in
de daarvoor bestemde vakken. De werkbegeleider geeft geen cijfer op dit formulier.
● Student zorgt ervoor dat dit getekende formulier opgenomen wordt in OnStage.
Eindevaluatie:
● Voor de eindevaluatie vult de student dit evaluatieformulier aan en uploadt het in
Onstage als onderdeel van het portfolio.
● Student stuurt de link van het eindbeoordelingsformulier naar de werkbegeleider (zie
HUbl). De werkbegeleider vult het formulier digitaal in.
● Het cijfer komt tot stand door het gemiddelde van de cijfers van alle CanMEDSrollen te
berekenen. Dat gebeurt automatisch wanneer het formulier in Formdesk wordt ingevuld.
Alle rollen wegen daarin even zwaar. Wanneer één van de beoordelingen onvoldoende is
wordt de stage onvoldoende beoordeeld.
