Charting (Upper right 2nd molar)-
Alphanumeric- UR7
FDI- 17
Class II Division 1 Malocclusion- Open lip posture
Class II Division 2 Malocclusion- Upper incisors trapped behind lower lip
Class III Malocclusion- Mandible too far forward
Jaw Relationship:
Class I- Ideal relationship
Class II Division 1- Increased overjet, lower lip trapped behind upper
incisors
Class II Division 2- Decreased overjet, increased overbite of upper 1’s
Angle’s Classification- Relationship of each first molar’s (occlusion/ adjacent
first molars)
Thumb Sucking- Decreased overbite and increased overjet
BPE:
0- Healthy
1- Bleeding on probing
2- Supra-/ Subgingival calculus, plaque retention
3- Pocket up to 5.5mm
4- Pocket- Deeper than 5.5mm
*- Furcation present
Tooth Mobility:
1- Side to side movement less than 2mm
2- Movement more than 2mm
Vertical movement
