Assessment and Treatment Plan

CBT/REBT is a form of therapy that believes the client's current issues stem from their

emotions and behavior from ideas, attitudes, beliefs, and thought processes. Assisting the client

in revealing the positive outlook in changing their thinking or looking at life circumstances leads

to an emotional change in their behavior. CBT/REBT's objective is to help the client make

positive changes that lead them to a more productive and fulfilling life. This assessment and

treatment plan comes from a counseling session of a Mexican American woman named Liz and

her therapist. This paper will assess the client's presenting problem and create a treatment plan

to improve her road to recovery.

Presenting problem

The information gathered from the client during the therapeutic session. The client is a

Mexican American female who reports she is in an abusive marriage with her husband. The

client suffers from lack of sleep, thoughts of unworthiness to her husband, or does not meet her

family expectations—the client's sleep disturbance reduces her energy. The client has low self-

esteem, feels that she will be following in her mother's footsteps. She thinks that she has no

option out of her situation and thoughts of uncertainty and hopelessness. The client discloses

witnessing her father abusing her mother. The client has lost weight due to a loss of appetite.

The client mentions suicidal ideation, feels her children and family would be better off without

her. The client expresses thoughts of uncertainty about her marriage and family. The client gets

angry at herself when she reflects on her relationship with her husband. The client's family

beliefs and values are "you stay married once you get married." The client does not want to end

, up alone like her mother. The client's "hopes of Change" show signs of her making her first step

for progress and recovery.

History of the Problem

The client reports she has a nine-year-old son and a seven-year-old girl, and she has

been married for a least 10-years with her husband (Robert); about two months ago, she reveals

when it all happened. The client states, "he would push and slap her on occasion." The client

and her husband would argue with each other. The client feels her husband's drinking habits

increased. She confronted him about his drinking habit, which created her husband verbally

abusing the client with harsh and inappropriate words. The client's husband continues

threatening, stating "that he would kill her, hurt her, and slap her" in front of their children.

Client children witness the father psychically and verbally abusing her. The client was scared

and relocated to her parent's home and now stays with her parents. The client states, "she avoids

her husband's calls because she believes he would convince her into coming back. The client is

protective and loves her children and mother. The client thinks she loves her husband. The

client shows courage to want to change or turn her life around. The client offers "hope" and

believes that her marriage can change for the better. The client says she was taught to stand up

for what they believe. The client's support system gives her strength. The client is willing to

have a second counseling appointment. The client's family beliefs and values are that once you

get married, you stay married—it is causing distress to the client's feelings of helplessness. The

client fears she only has two options: to return to an abusive marriage or turn out like her

mother alone. The client speaks of suicidal ideation, feels her children and family would be

better off without her.

Social and Family History

The client has a family history of abuse; the client witnesses her father's abuse of her

mother. The client revealed, her mother would not come out of her room because of her
