Soorten chemotherapie
• Curatief = genezend
-> voor tumoren die gevoelig zijn voor cytostatica
• Adjuvant = na een locoregionale behandeling
-> nog niet-aantoonbare, microscopische metastasen ook aanpakken
• Neoadjuvant = voor een locoregionale behandeling om tumor te verkleinen
-> weefsel sparend opereren
• Concomitant = gelijktijdig met radiotherapie
-> radiosensitiserend effect van heel wat cytostatica betere overleving
• Palliatief = met als doel levensverlenging of vermindering klachten
Toxiciteit van cytostatica
• Niet-selectieve toxiciteit: biochemisch systeem van gezonde cel ≈ kankercel
ernstige toxiciteit op gezonde weefsels
• De meeste cytostatica beschadigen DNA hoofdzakelijk snel delende cellen getroffen:
- zekere specificiteit voor tumorcellen (delen snel)
- gezonde weefsels waar cel vernieuwing belangrijk is meest getroffen: haren, huid,
maag-darmkanaal, beenmerg, geslachtsorganen
• Zowel acute toxiciteit als lange termijn effecten
• Acute toxiciteit: dosisafhankelijk en reversibel
- Hematologische toxiciteit bepaalt de dosis en frequentie van therapie
- Therapie soms uitgesteld/dosis verminderd als sterk tekort aan bloedcellen/
bloedplaatjes
• Laattijdige toxiciteit: cumulatief en vaak irreversibel
• Patiënt-afhankelijk en geneesmiddel-afhankelijk

- leukocytopenie (tekort aan witte bloedcellen)

- thrombocytopenie (tekort aan bloedplaatjes)

- misselijkheid, braken en diarree
kan leiden tot uitputting, uitdroging, ondervoeding, depressiviteit
- verlies smaak en geur
door direct toxisch effect op reuk- en smaakreceptoren of
door droge mond, uitdroging neusslijmvliezen,…
• Haar-, huid- en nageltoxiciteit:
- alopecie (haaruitval)
haarfollikels komen in rustfase of toxisch effect op haarmatrixcellen
- fotosensitiviteit, verkleuring huid, hand-voetsyndroom
hand-voetsyndroom: roodheid, blaren, strakke huid op huiddelen met veel
wrijving
