4. Verloskundige parameters: baarmoeder en naweeën4.1 Baarmoederinvolutie en lochia
4.1.1 baarmoederinvolutie
Uterus = baarmoeder
Opgelet: volle blaas en vol rectum hebben (negatieve) invloed op stand fundus en involutie.
Involutie = terugkeren van bm fundushoogte naar toestand van voor zwangerschap.
Snelle involutie: eerstbarenden en borstvoeding
Tragere involutie: multiparae, meerlingen, polyhydramnion (te veel vruchtwater),…
Involutie te traag = subinvolutie
Blijft fundusstand hoog, oorzaken:
- Vulling van bm met bloed/klonters
- Verplaatsing bm door overvolle blaas of rectum
- Overrokken uterus
- Oververmoeide uterus (bv. langdurige bevalling)
- Infectie (endometritis) of placentarest
In deze gevallen kan je een uterotonicum geven zoals synthocinon (samentrekken, ontspannen bm) of
methergin (1 kramp, bm krimpt snel samen)
Contractietoestand
Veiligheidsbol/ bol van zekerheid = baarmoeder trekt na bevalling terug samen en voelt aan als een harde bol.
Na 6-8 weken heeft bm terug normale grootte zoals voor de zwangerschap.
4.1.2 Bloedverlies/lochia
Lochia = bloederige afscheiding uit vagina
Parameters van lochia: hoeveelheid, aard/aspect, geur en kleur
Hoeveelheid
Normale hoeveelheid bloedverlies tijdens partus = 200 à 400ml
Indien meer of minder onderscheiden we:
gering bloedverlies: wanneer opvallend weinig bloedverlies is opgetreden;
matig bloedverlies: wanneer de hoeveelheid zich aan de lage kant van het gemiddelde bevindt;
ruim bloedverlies: wanneer de hoeveelheid bloedverlies zich eerder aan de hoge kant van het
gemiddelde bevindt;
veel bloedverlies: wanneer het bloedverlies ca. 500 ml bedraagt;
(postpartum)bloeding = fluxus: wanneer het bloedverlies >500 ml (of >1000ml, naargelang de
definitie!) bedraagt;
Aard/aspect
Bloedklonters mag, zeker eerste dagen.
Elke dag minstens 3x verloskundige parameters gecontroleerd.
Geur
Bij fletse geur of visgeur: evt ontsteking
Normale geur als alles in orde is
Kleur
Normaal = fel rood en na tijdje wat bruiner.
