, Samenvatting
Voor mijn keuzedeel jeugd en opvoedhulp heb ik een procesverslag geschreven over een jeugdige en
zijn systeem die ik heb ondersteund. Bij de ondersteuning heb ik een ondersteuningsplan gemaakt. Ik
heb hierbij gekozen voor een cliënt die bij mijn stageplaats in traject is. Cliënt heeft meerdere
problematiek waar aan gewerkt moest worden. Daarbij heb ik de moeder van cliënt ondersteund in de
opvoeding en ondersteuning afgestemd met de school van cliënt.
In het eerste deel van mijn verslag zal ik wat meer vertellen over de situatie van de cliënt om alvast
een beeld te schetsen voor de lezer. Hierbij wordt duidelijk wat de problematiek is waar cliënt en
systeem tegenaan loopt. Vervolgens komt het procesverslag aan de pas, hierin zal ik antwoord geven
op 18 vragen om het proces wat ik samen met cliënt en zijn systeem heb doorlopen toe te lichten. Tot
slot komt het ondersteuningsplan aan bod. Het ondersteuningsplan is gericht op de hulpvragen van
cliënt en systeem en daarbij de aanpak van de doelen waar cliënt en systeem aan moeten werken.
