Wat is moe?
Algehele malaise/zwakte - moeite om activiteiten te starten
Verminderde inspanningstolerantie – moeite om activiteiten vol te houden
Mentale vermoeidheid – concentratie – en geheugenstoornissen, emotionele labiliteit
Slaperigheid, kortademigheid, spierzwakte
Onderliggende oorzaak:

o Psychiatrisch – stemmingsstoornis, angststoornis, middelenmisbruik
o Somatisch – anemie, DM, hypothyreoïdie, hypocortisolisme, hemochromatose,
maligniteit, chronisch nierfalen, infectieziekten (HIV, TBC), auto-immuunziekten
(hepatitis), systeemziekten (sarcoidose), hartfalen, COPD, obstructief
slaapapneusyndroom, neurologisch (ALS), geneesmiddelen
Somatisch Onvoldoende verklaarde Lichamelijke Klachten (SOLK)
Anamnese: begin en beloop. Dagelijks patroon (continu, dagschommelingen). Uitlokkende factoren.
Verergering door inspanning, verbetering door rust. Slaappatroon (in-, door- en uitslapen, snurken,
apneu). Aanpassingen in dagelijks functioneren. VG. Geneesmiddelen.
Richt je op begeleidende verschijnselen (volledige tractus anamnese):
Vermoeidheid is bij somatische aandoeningen zelden het enige symptoom.
Aanvullende diagnostische clues vaak specifieker dan vermoeidheid.
Alarmsymptomen: gewichtsverlies, nachtzweten, lymfadenopathie.
LO: vitale parameters, lymfeklierstations, schildklier, hart- en longen, oriënterend neurologisch
onderzoek, buikonderzoek. Op basis van klachten gericht aanvullen: orthostase, gewrichtsonderzoek.
AO: wordt vaak wel gedaan.
Bloedonderzoek: volledig bloedbeeld incl differentiatie, elektrolyten, glucose, nierfunctie,
levertesten, TSH, CK (rhabdomyolyse, spierziekten), ferritine (anemie, hemochromatose), BSE.
Op indicatie aanvullen: serologie, ECG, X-thorax, cortisol.
Risico's van (over)diagnostiek: vals positieve uitslagen, kosten, vertraging in diagnose
Bayesiaanse statistiek: a priori kans -> diagnostische test -> a posteriori kans
Chronisch vermoeidheidssyndroom: gevoel van zwakte, energiegebrek of motivatiegebrek. Niet gevolg
van inspanning. Niet verbeterend met slaap of episode van rust. 50% beperking van activiteit. >6
maanden. Geen ziekte die vermoeidheid kan veroorzaken. Vaak begeleidende symptomen: keelpijn,
artralgieën, pijnlijke cervicale of axillaire klieren, hoofdpijn, orthostatische intolerantie. Somatisch
Onvoldoende verklaarde Lichamelijke Klachten.
