hoofdpijn)
1. Definitie
 Hoofdpijn = pijn of ongemak tussen de banen en het achterhoofd, veroorzaakt door
pijngevoelige structuren
 Het is een symptoom met verschillende oorzaken en is geen diagnose. De
verschillende oorzaken zijn
o Neurologisch
o Psychologisch
o Otologisch = vanuit het oor
o Opthalmologisch = vanuit de ogen
o Systematisch
 5-10% van de bevolking consulteert regelmatig een arts, vrouwen doen dit vaker

2. Pijn-gevoelige structuren
 Intracraniaal = veneuze sinussen, corticale aders, basale arteriën, dure van
anterieure, middelste en posterieure fossae
 Extracraniaal = hoofdhuid vaten en spieren, orbitale inhoud, slijmvliezen van
(para)nasale ruimte (neusruimte), uitwendig- en middenoor, tanden en tandvlees
 Referred pain = pijn die wordt gevoeld op een plaats, terwijl die ergens anders
ontstaat
 Zoals temporo-mandibulaire dysfunctie: het kaakgewricht
 Dura mater van de convexiteit en hersenen niet pijngevoelig
 Hersenoperaties kunnen dus onder plaatselijke verdoving uitgevoerd worden
(plaatselijke verdoving van huid en periost).
 Voorste en middelste schedelgroeve: V, achterste schedelgroeve: IX, X. Als hoofdpijn:
tractie of prikkeling van hersenvliezen of bloedvaten. Meningeale prikkeling door
ontstekingsinfiltraat, uremie, afbraakproducten van bloed of door ruimte-innemende
processen

3. Classificatie
 Primair of secundair
 Extracraniaal of intracraniaal
 Acuut of subacuut of chronisch
 Goedaardig of kwaadaardig
 Kan belangrijk symptoom zijn van een ernstige intracraniële aandoening, maar meestal
zijn er dan andere symptomen aan voorafgegaan. Daartegenover ook veel mensen waarbij
nooit een duidelijke oorzaak gevonden wordt.

Er wordt onderscheidt gemaakt tussen:
 Primaire hoofdpijn
 Secundaire hoofdpijn (etologisch criterium)
 Craniale neuralgieën en centrale of primaire aangezichtspijn en andere hoofdpijn

4.1. Primaire hoofdpijn (ICHD II)
 Migraine
o Migraine zonder aura (aura = voorbode van dat er iets gaat komen)
o Migraine met aura

 Spanningshoofdpijn
o Onregelmatige episodische spanningshoofdpijn

 Wakker worden door hoofdpijn vlak na inslapen
 Geen migraine symptomen
 Kloppend uni- of bilateraal
o Primaire donderklaphoofdpijn
o Hemicrania continua = hoofdpijn continu aan 1 kant
