Inhoud en voorbereiding
- Voorbereiding;
o Handboek oncologie: hoofdstuk 17;
- Per tumorsoort;
o Cervixcarcinoom;
o Endometriumcarcinoom;
o Vulvacarcinoom;
o Ovariumcarcinoom;
- Wordt besproken:
o Hoe wordt de diagnose gesteld;
o Wat is de behandeling;
o Wat is de uitkomst/ prognose en hoe gaat de follow-up.
Cervixcarcinoom
- Incidentie 800 per jaar;
- Piek 30-44 jaar.
Klachten:
- Irregulair vaginaal bloedverlies;
- Postcoïtaal (na geslachtsgemeenschap) bloedverlies;
- Aanhoudende, veranderde vaginale afscheiding;
- Of geen klachten en afwijkende cervix smear.
Cervixcarcinoom – diagnostiek
- Colposcopie en LLETZ (=lisexcisie) bij premaligniteit;
- Labonderzoek (Hb, nierfunctie, scc (marker voor plaveiselcelcarcinoom));
o Nierfunctie is belangrijk voor het stadium van de ziekte. Op het moment dat de
tumor al uitgebreid is, druk het de urineleiders dicht wat uiteindelijk hydronefrose
ontstaat.
- Recto-vaginaal toucher;
- MRI-scan;
o Om te kijken of er uitbreiding is naar de lymfeklieren eromheen.
- Indien verdenking op uitgebreide metastasering zoals bijvoorbeeld aangedane lymfeklieren
dan ook CT thorax/ abdomen.
