Inleiding
Tijdens de stage op afdeling P, cardiologie, in het Medisch Centrum Leeuwarden (MCL)
wordt er gewerkt aan de module werken aan gezondheid. De module richt zich op het
bevorderen van de gezondheid, participatie maar ook het zelfmanagement van de patiënt.
Een veel voorkomend probleem op de afdeling is decompensatio cordis. Dit ziektebeeld zal
centraal staan tijdens deze module en er wordt gekeken naar de mogelijke preventiezorg.
Om een beeld te creëren over preventie wordt hier de benodigde informatie over verkregen,
na deze informatie kan er een compleet preventieplan worden ontwikkeld.

De module wordt doorlopen aan de hand van de 11 stappen van het model van Rosendal, de
11 stappen zijn:
