NUR 3225 Acute and Chronic Test 1 Study Guide Test items will be included which ask you to delegate nursing interventions. LPNs may not assess.  The sickest patients (most unstable, most acute) should have experienced RNs caring for them.  The most stable (least sickest, least acute) patients may be delegated to float RNs who have less experience.  The RN should assess the sickest (most unstable, most acute) patients first.  An RN may delegate tasks to nurse externs/techs such as vital signs, emptying urine output from a catheter or urinal, adding up intake and output, bathing, changing linens, applying electrodes on a telemetry floor, assisting with eating, ambulating, and documenting their care. As unlicensed personnel (UAPs), they may NOT make assessments, interpret data, make judgments, take physician's orders.  Patients with the highest need for monitoring/highly technical equipment should be taken by experienced RNs. This could mean patients with monitoring devices such as PA catheters, arterial lines, patients on dialysis, patients on mechanical ventilation, etc.  Review acuity:  Patients very recently admitted (within past 2 hours for example) are usually sicker (more acute) than those preparing for transfer to another unit or for discharge home.  Patients with an underlying chronic disease are often more complex (more acute) than a healthy young adult with an acute illness episode.  Very young and elderly patients are often sicker than the young to middle age group. Most of the objectives are detailed and specific, but for those that are not,…….I gave examples as well as provided more focus, hope this is helpful and of course, this study guide should not replace your usual study methods, it is meant to augment.
