a.Inleiding
- Duizeligheid/vertigo
o Zeer veel voorkomende klacht (1/10 bij huisarts of neuroloog)
o Aspecifieke klacht
o Definiëring
Duizeligheid = algemene term voor een gevoel van desoriëntatie
Vertigo = illusie dat er beweging plaatsvindt van de persoon of van de
omgeving (vaak rotatoir van aard) ‘spinning sensation’
o Verschillende vormen/oorzaken vertigo
Vestibulaire oorsprong
Draaiduizeligheid
Perifeer/centraal
(BPPV is probleem in perifeer vestibulair apparaat)
- BPPV = werkelijke vertigo

Anterieur kanaal
o 2 otolietorganen:
Utriculus
Sacculus
Hieruit vertrekt de N.
vestibulocochlearis (de gehoor- en
evenwichtszenuw, N. VIII) =
verantwoordelijk voor de overdracht van
geluid en evenwichtsinformatie van het
binnenoor naar de hersenstam
(vestibulaire nuclei), cortex & cerebellum
(ex!!) Connectie naar kernen die oogspieren innerveren N. oculomotorius (N. III), N.
trochlearis (N. IV) en N. abducens (N. VI)
b.1. Otolietorganen
- Werking
o Zijn gevuld met vloeistof en zintuigcellen (bakercellen?)
o Op de zintuigcellen liggen trilhaartjes met kleine oorsteentjes (kristallen)
o De zintuigcellen nemen drukveranderingen van deze steentjes waar (als we met ons
hoofd bewegen, bewegen de trilhaartjes mee) afbuiging zorgt voor een elektrisch
signaal (AP) die via de N. vestibularis verstuurd wordt
- Functie
o Hoofdbewegingen: registratie van lineaire versnellingen (bv. in een lift of een auto)
