This is Unit 5 Task 2, including Learning outcome D. This assignment is graded distinction.
Unit 5: Learning outcome D- Investigate the roles of professionals and how they work together to
provide the care and support necessary to meet individual needs.
In this assignment, I will be writing about Unit 5 outcome D, where I talk about multi-disciplinary
teams and multi agencies. I will talk about maintaining confidentiality and different legislation that
protects service users. I will link three case studies to each section and explain how it can be linked.
D1- how agencies work together to meet individual care and support needs.
In this section I will talk about how multi agencies meet people needs. Multi agencies are
involvement of professional and support staff from a range of bodies such as social services,
education or healthcare (medical dictionary N.D). There are many organisations that commission
healthcare services, which are:
Clinical commissioning groups, this was form in April 2013 and the roles are giving patients,
communities or clinicians more choice in local services. These groups involve GPs and other health
care professionals. For example, they decide what is needed in a local area, like stroke-on-tent has
the highest rate of teenage pregnancy, clinical commission group would target this and bring more
contraceptive in (NHS clinical commission group N.D). Local health boards in wales, formed in 2009
and improves healthcare in NHS wales, this provides care closer to people. They are responsible for
improving health of patients and promoting wellbeing (NHS wales health board 14/08/20). Members
include GPs, nurses, dentist or public relations and finance. Health and social care board in Northern
Ireland, formed in April 2009 and improves management of health and social care services.
Members include various health and social care services, finance and public relations.
Local authorities promote health and wellbeing and work close to organisations like the NHS and
housing. Local authorities are responsible for commissioning and provides funding. Local authorities
will need to come up with a strategy for their people to improve health (TheKingFund commissioning
19/08/19).
Roles of bodies responsible for integrating health and social care, an example if HWB (health and
wellbeing boards N.D). This was introduced in 2012 and the aim was accessing the needs of the
population, reducing inequality, new strategies and more mutli disciplinary teams working together.
HWB works closely with CCGs to make an assessment for the needs of the population.
Role of assessment and eligibility frameworks are used within health and social car. Framework
means ways to evaluate needs where eligibility is who uses these needs. Frameworks in health and
social care lead to improving health. Common assessment framework uses in children services and
asses’ children with additional needs. The plan is to identify needs early and build a mutli disciplinary
team around them (CAF Thurrock.gov N.D). The national eligibity criteria is a single framework that
assess care and support needs, it is section 13 out of the care act. This supports the needs and make
sure they meet the needs (The Care Act 2014 N.D). Department of health is a part of the government
that focuses on health, they assess the public health. The national framework for NHS continuing
healthcare, this sets out principles on how to continue NHS plan. An individual can be eligible for this
if they have a primary health need. This means the NHS is responsible for care and their health and
social needs. This framework includes the care act, CCGs. The framework is based on the patient
needs and they are free from discrimination (national framework for NHS continuing healthcare
March 2018 n.d).
, The education, health and care plan are for children and young people who need support or extra
help. This means they have a support plan for these children. This can be planned by someone from
school or a parent. The local authority will carry out an assessment for the child to help them get the
right help needed.
P6- explain why meeting the needs of the individuals requires the involvement of different
agencies.
By meeting the needs of people, we will need involvement of different agencies to help them
improve. This can be showed through the case studies:
In case study one, Patrick have worked with a multi-disciplinary team which are sexual health
adviser, counsellor and G.P. They also work in a multi-agency team where a housing officer involved
and their trainer. These type of teams helps meets the needs of Patrick and James by improving their
health and wellbeing. Patrick and James can benefit from different agencies as they are in favour of
looking after their health. When they were transferred to their G.P they were able to give up with a
treatment plan and a multi-disciplinary team of counsellor and a nutritionist. This team will help
them improve their lifestyle and future health. Patrick and James needs were met by having
different agencies without this means their needs would not be met. For example, without having
the involvement of the G.P Patrick and James would not have had this plan and their future health
could get worse. This case study requires help from different agencies to help improve Patrick and
James lifestyle, each agency or professional has a different role to play in helping. This is a successful
way of using multi-disciplinary or multi-agency teams. Linking back to D1, clinical commission group
could look at this case and see how high the local STD chances are, if they are high, they can
introduce more sex clinics or more prevention. Maybe if this was put in place before Patrick and
James might not have gotten HIV. This is a concern to CCGs to bring in more awareness of wearing
condoms. The local authority could improve the funding for STD awareness or more care services.
The local authority has worked with Patrick and James within housing (multi agency team). Within
their multi-disciplinary team, they have a counsellor to improve their wellbeing. This team works
well by keeping information confidential and gives the right help needed. The Terence Higgins trust
is a British charity that deals with HIV and sexual health, this can link to CCGs where its giving people
more choice in the communities. Patrick and James have worked with this charity which will give
support and advice for them. Their local HWB would work closely with HWB to improve the sexual
diseases around the around Patrick and James live in for the future cases.
In case study two, Brenda has a mutli agency team around her including health visitor, nurse, G.P.
manager and an advocate. All these professionals are from different agency however they are
working together to get Brenda the best care possible. This team worked together to help Brenda
needs are met and each professional had their own roles. Without this team Brenda can lack care,
for example her health may get worse as she lacked a certain professional. Her team is mutli agency
which means professionals from different agencies, Brenda has a manager and an advocate which
are not health professionals but are there to help her understand and speak for her. Her team need
to communicate effectively for Brenda to get her needs met and for effective care. The local
authority would need to be funding into Brenda’s case and come up a plan for her to live
independently like her sheltered housing. Brenda may be eligible for the national framework for NHS
continuing, this can help her and reduce discrimination as Brenda is vulnerable. Brenda can also be
under the national criteria ensuring her care, this is also under the care act. Their local CCGs and
HWB would work closely together to ensure the help for future cases, for example if Brenda area has
a high level of mental health or asthma, they would work together to solve this.
