MO Mondopening
rp Rustperiode: boven- en ondertanden raken elkaar niet
h De beweging die mandibula beschrijft bij MO vanuit rp
ICP Intercuspidale contactpositie (centrale occlusie): tanden op elkaar
PP Maximaal geprotrudeerde contact positie = protrusieve occlusie
RCP Maximaal geretrudeerde contact positie = retrusieve occlusie
- ROM van ICP naar RCP = 7-10mm
- ROM van ICP naar PP = 0-22 mm
- PP —> MO = beweging die gemaakt wordt als je kaak naar voor
doet (PP) en dan mond opent
- ICP —> MO = normale opening. Met je tanden maximaal op
elkaar beginnend
- RCP —> MO = uit maximale retrusie (kaak naar achter) je mond
open doen
- PP —> RCP = je kan niet in een rechte lijn van retrusie naar
protrusie. Komt door je overbeet
o Overbeet = bovenste snijtanden staan net iets over je onderste snijtanden

3.1. Articulair
- Temporomandibulair gewricht: TMG

- Linker en rechter TMG bewegen altijd samen (door dat os mandibula elkaar verbindt)
- 145-160° hoek tussen de gewrichtsassen
TMG
- Fossa articularis + tuberculum articulare van het os temporale = concaaf
o Fossa = groter dan je caput
- Caput mandibulae (kaakkopje) = convex
o Maar is discongruent aan de concave fossa
- Discongruentie opvangen door intra-articulaire discus (= 3 e gewrichtspartner van
TMG)
- TMG omringd door kapsel: binnenzijde bedekt door synoviaal membraam dat het
vocht afscheidt
o = glijmiddel voor het gewricht
- MPTL
o Superior: hecht aan op de discus
o Inferior: hecht aan op caput mandibulae
Discus
- Intra-articulaire discus
- Collageen bindweefsel met elastische vezels
- Biconcaaf (zowel boven als onderaan een concaviteit) met bovenkant zadelvormig
- Volledig rondom verbonden met gewrichtskapsel
- TMG wordt dus in 2 compartimenten onderverdeeld
o Bovenste deel = tussen fossa art, tuberculum art en discus
o Onderste deel = tussen discus en caput

Onderverdeling discus
- V = Voorste band (pes meniscus) —> vezels van M.
Pterygoideus lateralis pars post hechten hierop aan
- I = Intermediaire zone (pars gracilis)
- A = Achterste band (pars posterior)
- Bilaminaire zone met
o Bi = Stratum inferior
o Bs = Stratum superior
o = losmazig bindweefsel dat bevloeiing en bezenuwing van discus
o Discus beweegt mee door elastische bindweefsels, niet door spieren

Palpatie caput:
- Net onder jukbeenderen, tegen je oor
- Als je mond opent, voel je ze tegen je vingers schieten
- Komt door een te vroege anterieure translatie
1. OPENEN
- Onderscheid maximale opening en functionele mondopening
o Max MMO = 53-59 mm
o Functionele = 70-80% van de MMO
- Verdeeld in 2 fases
- Begint in onderste compartiment (caput) en gaat dan naar bovenste (discus). Sluiten
is omgekeerd
Fase 1: rotatie
- Enkel in onderste compartiment van TMG
- Zuivere rotatie van caput mandibulae t.o.v. discus
- Staat in voor 50% van functionele MO
- Gebeurt met ontspannen M. Pterygoideus lat pars sup
- Caput mandibulae gaat nog niet naar voor
Fase 2: Translatie
- In bovenste compartiment van TMG
- Ventrale translatie van condylus en discus langs helling van tuberculum articulare
- Mogelijk door contractie van MPLs
- Hoe verder je de mond opent, elastine rekt mee
Sluiten
- Superieure kop MTP blijft opgespannen
- = excentrische contractie om elastine niet te snel te lossen
- MTPL inferior: zorgt dat caput weer naar achter gaat (is ontspannen nu)
