- Vanuit coronair takjes alleen tijdens diastole ( want knijpkracht hartwant = systole kracht:
netto geen druk) alle andere organen tijdens diastole en systole
- Bij hartfrequentie verhoging: systolische duur blijft even lang: diastolische tijd verkort.
- 60/min is de optimale frequentie voor doorbloeding hartspier
- Bij HF 60/min: diast:syst: 2:1 doorbloeding 40 sec/min
- Bij HF 90/min: diast:syst: 1:1 doorbloeding 30sec/min
- Bij HF 120 per min: diast:syst: 1:2 doorbloeding 20sec/min

Angina pectoris
Wordt gekenmerkt door de volgende 3 symptomen:

- Pijn op de borst (retrosternale klachten, beklemming, drukkend, samensnoerend gevoel op
de borst  kan tevens uitstralen naar kaak, arm, rug en/of dyspnoe veroorzaken)
- Uitlokken klachten door inspanning of emoties
- Verdwijnen van klachten in rust en/of door sublinguale nitraten binnen 2-15 minuten

Let op: bij ouderen, mensen met diabetes en vrouwen kunnen de symptomen minder duidelijk zijn.

Anamnese

- Uitlokkende factoren (Hartfrequentie en HMV verhoging) vaak herhalend bij vergelijkbare
inspanning
- Hartspier O2 behoefte stijgt + 02 toevoer daalt
- Duur van de klachten (bij stabiele AP duren klachten in de regel minutenlang, max. 5-15min)
reagerend op rust en/of nitro
- Dit maakt stabiele AP waarschijnlijk: extra aanwijzing - bestaande risicofactoren,
bestaande atherosclerose

Indien teven: begeleidende vegetatieve verschijnselen zoals zweten, misselijkheid, bleek gelaat,
angst, onrust of langer aanhouden dan eerder myocard infarct. Pleit voor ernstige ischemie (ACS)
ofwel IAP of AMI

Lichamelijk onderzoek

- Polsslag: frequentie (en ritme)
- Bloeddruk  verhoogd: risicofactor + hogere belasting
- Kleur: bleek  anemie, cyanose
- Auscultatie hart: frequentie, ritme
- Souffles  = geruisen
- Auscultatie longen  crepitaties passend bij ontstaan hartfalen, longziekten (inspiratie,
expiratie, piepen etc. vooral als DD (longembolie, pleuritis)

Etiologie en risicofactoren

- Coronairlijden (atherosclerose)
- Andere bewijzen atherosclerose vaatziekten
- Evt. gecombineerd met anemie, ritmestoornissen
- Hypertensie, roken, hypercholesteroleie, obesitas, familiair, man, diabetes mellitus

Aanvullende diagnostiek

- Inspanningstest (ergometrie)
- Stress ECHO  verminderde bewegingen hart
