1. ADHESIE
= vastkleven van plaatjes aan niet-plaatjesoppervlakken in tegenstelling tot
aggregatie
Vindt plaats op beschadigde vaatendotheel waar het subendotheel
vrijkomt en er binding kan doorgaan tussen plaatjes en het
vrijgekomen collageen
VWF is noodzakelijk, dat zal binden op GP Ib en collageen, daardoor vormt
een verbindingsproduct
VWF = reusachtige multimeer die ook kan binden op GP IIb/IIIa, enkel als de
bloedplaatjes geactiveerd zijn
Fibrinonectine heft een gelijkaardige functie als VWF maar wordt in veel
minder mate aangewend bij het adhesie proces
2. VORMVERANDERING EN PROSTAGLANDINESTYNTHESE
Bij adhesie veranderen plaatjes van discoïd naar sfeervormig met fijne uitlopers (filipodia) + er zullen
prostaglandines gevormd worden die het aggregatieproces zullen bevorderen
TXA2 wordt gevormd uit PGG2/PGH2 (PGG2 heeft zeer korte halfwaardetijd en wordt omgezet tot
PGH2 door reductie)
PGG2 wordt gevormd uit arachidonzuur mbv clycoöxygenase
TXA2 is zeer labiel. Het is een sterke plaatjesactivator +
vasoconstrictor
PGI2 (=prostacycline) bevindt zich in de vaatwand en wordt gevormd
uit PGH2 mbv prostacyclinesynthetase
inhibeert de plaatjesaggregatie en stimuleert adenylaat cyclase
met vorming van c-AMP, waardoor Ca terug in de opslagplaatsen wordt gedreven en de
plaatjesaggregatie vermindert + veroorzaakt vasodialatie
= ANTI-AGGREGEREND EFFECT
