Abstract
There is a lack of knowledge for CHF patients newly diagnosed patients and
patients with repeated admissions. Congestive heart failure is a complex diagnosis that requires
patient education for patients to perform high quality self-care. To help educate patients,
improve quality outcomes and reduce re-admission rates a nurse led CHF education program is
proposed for inpatients that are admitted to the hospital. The proposal recommended that one
nurse be hired initially and 3 additional nurses were to be hired as the program demonstrated a
successful reduction in re-admission rates. The CHF discharge nurses were charged with
responsibility of seeing all newly diagnosed and re-admitted patients diagnosed with CHF. The
education proposal would start on the first day of the patient admission. Daily educational
sessions were proposed to be held until the patient was to be discharged. The plan of action was
to introduce and hand over the developed proposed practice change. To accomplish this a step by
step approach was taken. Step one was meeting with the mangers to seek approval for the
proposal. Once the managers gave their consent, step two was an evidenced based research
search was performed. The evidence based search consisted of 30 peer reviewed journal articles
focused improving CHF readmission rates and improving outcomes. Best practices were
identified through the compilation of research. The evidence was presented to the stakeholders.
Step three involved meeting with Human Resources for budget related items and hiring new
employees. In step four the stakeholders were consulted and given the opportunity to give input.
Evidence was reviewed with the stakeholders that showed that a nurse-led CHF discharge
education program could increase quality outcomes and reduce re-admission rates. The budget
and feasibility of the proposal was discussed with stakeholders. The stakeholders gave their

was handed to the organization for implementation.
