Sinds kort heeft dhr. ook licht ontvelde plekken op zijn stuit en hak. Hij geeft ook aan
dat deze plekken pijnlijk zijn.
Taak
De taak is om het doel te bereiken die we voor ogen hebben: De komende twee
weken wordt voorkomen dat de decubitus op de hak en stuit van dhr. verergert
naar decubitus categorie III* (Kennisplein Zorg voor Beter, 2021). Met daarbij de
taak om te streven naar goede kwaliteit van zorg met daarbij gebruik te maken van
interprofessioneel samen te werken.
