↳ Gecompliceerd = kroon gefractureerd met pulpa-exponatie
Spalk periode is in Nederland 4 weken, internationaal 2 weken.

II. Vitale pulpa therapie
III. Autotransplantatie
Multidisciplinaire aanpak is zeer belangrijk.
● Pedo
● Ortho
● Chirurgie
Termen:
1) Partiële pulpotomie
↪ gedeeltelijke pulpa verwijderen uit de pulpakamer (tot aan de kanaalopeningen)
2) Volledige pulpotomie
↪ gehele pulpaweefsel uit de pulpakamer verwijderen, niet verder dan opening kanalen
3) Pulpectomie
↪ ook in de kanalen wordt het pulpaweefsel verwijderd
4) Wortelkanaalbehandeling
↪ hetzelfde als pulpectomie, maar dit is het gehele proces incl. irrigatie en obturatie etc.
Een pulpectomie is de eerste stap van een wortelkanaalbehandeling. Een pulpotomie wordt niet
vervolgd met een wortelkanaalbehandeling.
Ongecompliceerde kroonfractuur
Pulpanecrose komt weinig voor, maar is sterk afhankelijk van
○ Overig tandletsel
○ Soort behandeling
○ Fase wortelontwikkeling
Gecompliceerde kroonfractuur
Behandeling:
❖ Directe pulpa-overkapping
❖ (Partiële) pulpotomie

❖ Pulpectomie, alleen als de gehele pulpa is geïnfecteerd
➢ Alleen als:
■ Bloeding pulpa niet onder controle kan worden gehouden
■ Er ernstig bijkomend letsel aan elementen met afgevormde wortels is
Wortelfractuur
Vaak alleen te zien op röntgenfoto’s
Behandeling:
❖ Repositie, z.s.m.
❖ Semi-rigide spalk voor 4 weken maximaal
Horizontale wortelfractuur
Vaak na trauma
Bij een fractuur doe je, indien nodig, een endo tot de fractuurlijn.
Luxatie
Subluxatie = beetje los maar niet verplaatst
Lateraal = tand opzij verplaatst
Extrusie = naar buiten
Intrusie = naar binnen. Ernstige soort tandletsel door aantasting bot
Prognose is afhankelijk van schade aan:
● Parodontaal ligament
● Pulpa
Behandeling:
❖ Repositie, z.s.m.
❖ Semi rigide spalk (2-4 weken, fysiologische beweging is belangrijk!)
❖ Wortelkanaalbehandeling binnen 2-3 weken na trauma bij gesloten apex
➢ Om resorptie te voorkomen
Mogelijke complicaties
1. Oppervlakte-resorptie
↪ stopt vanzelf → geen behandeling nodig. Ruwig worteloppervlak maar geen radiolucenties.
2. Ontstekingsresorptie
↪ oorzaak: pulpanecrose, met bacteriën die lekken naar het pdl ⇒ wkb nodig. Ruwig worteloppervlak
met radiolucenties.
3. Vervangingsresorptie (ankylose)
↪ geen radiolucentie te zien. Schade aan pdl en niet aan pulpa dus wkb geen nut. Je ziet juist bot in de
plaats van de tand komen. Je kan dit niet behandelen.
Fysiologische beweging kan ankylose voorkomen.
4. Volledige obliteratie kanaal
↪ verkleuring tand, oorzaak: laterale luxatie
