Les 1: Fysiologische elementen

1 Bloedhersenbarrière (en bloed-CSV barrière)
Doel
• Bescherming CZS tegen schadelijke moleculen
o Actief transport door E-dependente kanalen
▪ Daarom bevatten cerebrale endotheelcellen zoveel mitochondria → energie
voor actief transport
o Diffusie // vetoplosbaarheid
o Capillairen hebben lage permeabiliteit
• Isolatie (Elektrisch milieu)
o Capillairen hebben hoge elektrische weerstand
Gevormd door
• Tight junctions in capillaire endotheelcellen
(+ in epitheelcellen die plexus choroideus bekleden)
Er bestaat ook een bloed-CSV barrière:
• Endotheel van plexus choroideus bevat veel
fenestraties → kan niet voor barrière zorgen
• Epitheel van capillairen van plexus choroideus heeft
tight junctions → barrière
Afb: capillair in hersenen met eindvoetjes daarrond
• Wand = endotheel
• Verbonden met tight junctions
• Transport: vooral actief
Verstoring BBB
• Weefseldisruptie / destructie
o Diffuus trauma
o Contusie
o Bloeding
o Ischemie
o Infectie
o Inflammatie
o Vrije radicalen, hyperosmolaire moleculen
• Disorganisatie
o Tumorale capillairen: sommige tumoren groeien zo snel dat de
capillairen slecht gevormd zijn en zwakke tight junctions hebben
o Foto: hersentumor met perifere contrastcaptatie
Dit zien we door contraststof in te spuiten → we zien lek.

,2 Hersenoedeem
Hersenoedeem = toename waterinhoud hersenweefsel
Dit kan op 2 plaatsen:
• Intracellulair
• Interstitiële ruimte
Er zijn verschillende vormen
• Vasogeen hersenoedeem
o In interstitium, witte stof
▪ Witte stof is minder compact dan grijze stof.
o Er lekt vocht uit capillairen: verhoogde capillaire permeabiliteit → verstoorde
barrière.
o Oorzaken
▪ Necrose (contusie) → capillairen zijn beschadigd
• Het oedeem dat hier bij komt, is ten dele
vasogeen, door disruptie en energie-depletie
van de endotheelcellen
▪ Neovascularisatie (maligne tumoren) → vaatjes w
zwak gevormd met slechte tight junctinos → lekken
o R/ cortisone gaat capillaire permeabiliteit verminderen.
▪ Dit werkt bij tumoren
▪ Werkt NIET bij trauma
o Afb: peritumoraal oedeem, in witte stof is er vlamvormig
oedeem
• Cytotoxisch hersenoedeem
o Intracellulair, grijze + witte stof → alle cellen zwellen op
o Oorzaken: beschadiging van cellen/neuronen/exonen
▪ Ischemisch CVA
▪ Leverfalen en uremie → toxines
▪ Traumatisch veralgemeend hersenoedeem door beschadiging van cellen
(diffuus) door bv acceleratie en decceleratie (auto-ongeluk,…)
o R/ Mannitol zorgt voor dalende intracraniële druk
▪ Zal vocht afdrijven uit cellen → naar interstitium → naar vasculaire
compartiment (via osmose)
▪ Kan bij alle gevallen van cytotoxisch oedeem gebruikt worden
o R/ Decompressieve cranioectomie
▪ Wegnemen van deel van schedel zodat schedel kan uitzetten
o R/ Corticosteroïden werken hier NIET
• Osmotisch hersenoedeem
o Intracellulair
o Oorzaken: pathofysiologische osmotische gradiënt: vocht van vasculair →
interstitieel → osmotische gradiënt schuift op → vochtshift naar intracellulair
▪ Daling van serumosmolaliteit
• Waterintoxicatie: osmolariteit in vasculaire compartiment daalt →
vocht in interstitium w aangetrokken
• SIADH: syndrome of inappropriate secretion of ADH
▪ Stijging weefselosmolaliteit: Resorberend hematoom
o Uiteindelijk zal water dat langs osmotische extra-intracellulaire gradiënt komt, ook
leiden tot cytotoxisch oedeem!

, • Hydrocefaal hersenoedeem
o Transependymaal transsudaat naar periventriculaire witte stof
o Oorzaak: bij forse obstructieve hydrocefalie (obstructie op circulatie
hersenvocht) → ventrikels gaan uitzetten → vocht gaat door ependym
transuderen
• Obstructief hersenoedeem
o Oorzaak: obstructie interstitiële flux naar veneus compartiment
▪ Veneuze sinustrombose
▪ Meningeoma
▪ Schwannoma
▪ Craniofaryngeomen
3 Cerebrospinaal vocht
Vorming
• Secretie, actief proces uit plexus choroideus
• (+ klein deel uit bulk flow interstitieel vocht uit parenchym)
• 0.3-0.4 ml / min = 0.5L/d
• ± 150ml CSV op elk ogenblik
o 30ml in ventrikels
o 120ml in subarachnoidale ruimte (75ml in spinale subarachnoidale ruimte)
Druk: 140-150 mmH2O of 10-11 mmHg
Samenstelling
• Helder
o Indien niet: probleem. Meningitis bv
• <5WBC/mm3
• 60% van serumglucose
• 0.4% van serumeiwit
Circulatie en absorptie
Circuleert vanuit laterale ventrikels → foramen van Monro →
3e ventrikel → via aquaduct naar 4e ventrikel Van hieruit van
foramen van Lushka en Magendie naar cisterna magna →
naar subarachnoidale ruimte
• Productie
o Plexus choroideus 3e V
o Plexus choroideus 4e V
o Plexus = gespecialiseerd epitheel dat de
capillairen bedekt
• Absorptie:
o Subarachnoidale ruimte → absorptie in
arachnoïdale villi (= uitstulpingen van
arachnoidea in veneuze sinussen van schedel)
o Ook door de venen rondom spinale zenuwen. Dura mater is heel dun.
o Absorptie gedreven door dynaemische drukgradiënt tussen subarachnoidale ruimte
en venezeu draingagesysteem
▪ Is het grootst tijdens systolische fase van hartcyclus
