Case Study


Mr. X is a 28 years old Caucasian male who presented to the ED with a complaint of an

acute onset of dyspnea and chest pain which started 2 hours ago. He reports that he was feeling

fine until about 2 hours ago when he began to suddenly feel short of breath. He reports a cough

without any sputum production, but has been having chills and feeling nauseated. His past

medical history is positive for HIV infection. He is on antiretroviral therapy and is reportedly

compliant with his medications. He has no other medical problems and a negative past surgical

history. His family history is negative and he has smoked 1 pack of cigarettes daily for the last 3

years. He drinks 6 beers daily and does not believe this is excessive ETOH use.

1. The clinical scenario is most consistent with which diagnosis?

 Community-acquired pneumonia (CAP)

2. What data in the clinical scenario supports your diagnosis?

 Acute onset of dyspnea and chest pain signifies CAP

 Tachypnea of 22 respiratory rate

 Lungs with rales right posterior base

 Cough without any sputum production

 Fever of 101.2. Fever is an adaptive host-defense response to infection through

destruction of temperature sensitive microorganisms.

 O2 sat is at 89% on room air. This can be attributed to V/Q mismatching.

 Patchy infiltrate in the right lower lobe from the accumulation of blood cells, fibrin,

edematous tissue, and pneumococci.
