Incidentie.
¼ zwangerschappen gepaard met bloedverlies.
15-20% van alle ZWS = miskraam.

Fysio-pathologie.
Bloeding in decidua basalis geeft aanleiding tot necrose & inflammatie. De gehavende zwangerschap
fungeert als vreemd lichaam & wekt contracties op.
Cervixdillatatie, expulsie in de lochia.
Hoe vroeger een miskraam optreedt, hoe meer kans men heeft op een volledige expulsie.
Indeling: vroeg – laat /Abortus completus – incompletus/… (zie handouts)
Volgens tijdsaspect:
Vroegtijdig miskraam.
= verlies v/d zw voor 14w.
Laattijdig miskraam.
= verlies v/d zw tussen 14 & 20w
(tss 20 & 26w => partus immaturus).
Volgens oorzaak:
Spontaan miskraam.
= zonder aantoonbare oorzaak.
Accidenteel miskraam.
= veroorzaakt door ongeval.
Therapeutische abortus.
AAP (abortus arte provocatus) = APLA (abortus provocatus (lege artis)).
= kunstmatig opgewekte abortus (artificiële abortus)

Herhaald miskraam.
= >2 spontane miskramen
Habituele abortus
= zich telkens herhalende abortus

Abortus completus
= complete abortus waarbij vrucht, vruchtzak en decidua intact zijn uitgedreven

= miskraam van tubaire zwangerschap, waarbij zwangerschapsproduct naar buikholte wordt uitgestoten.

Chr.afwijkingen (2/3), mola zw.sch.
Placenta-afwijkingen
Materneel:
Lokaal: congenitale afwijkingen, fibromen, cervicale incompetentie, endometritis
Algemeen: rhesusimmunisatie; infecties zoals listeria, syfilis, toxoplasmose
Endocrinologisch: diabetes, hyperthyroïdie, nefritis, corpus luteum insufficiëntie, geneesmiddelen
Immunologisch
Chronische aandoeningen - systeemziekte: lupus…
