Sulfonamides are commonly called sulfa drugs

 Ex: of Sulfonamides include: Sulfadiazine, sulfisoxazole, sulfamethizole
 They are often used to control infections caused by both gram-positive and gram negative bacteria, such as E-coli,
Staphylococcus aureus and Klebsiella and Enterobacter species

Uses: UTI, acute otitis media, ulcerative colitis.
Mafenide (Sulfamylon) and silver sulfadiazine (Silvadene) are topical sulfonamides used in the treatment and
prevention of infections in second and third degree burns.

Gastrointestinal System Reactions:
 Nausea, vomiting, anorexia, diarrhea, abdominal pain, stomatitis, chills, fever, crystalluria, photosensitivity
The most frequent adverse seen with topical application of a sulfonamide is a burning sensation or pain when the drug
is applied to the skin
Other possible allergic reactions include: rash, itching, edema and urticaria
Don’t take if younger than 2 years of age!
If sulfonamides are used with a pt with a renal impairment, hepatic impairment, or bronchial asthma it can make the
condition worse

Interacting drugs:

Oral Anticoagulants- Common use: Blood thinner; prevent clot formation. Effect of interaction: Increased action of the
anticoagulant

Methotrexate: Common Use: Immunosuppressant and chemotherapy.
Effect of interaction: Increased bone marrow suppression
Hydantoins: Common use: Anticonvulsants. Effect of Interaction: Increased serum hydantoin level
Nursing Diagnosis include:
 Impaired urinary elimination
 Impaired skin integrity related to burns
 Impaired skin integrity related to allergic reaction or photosensitivity
 Risks for secondary infection related to lowered WBCs

Pt teaching: Take the drug at prescribed time intervals

 Drink 6-8 oz glasses of fluid while taking these drugs and take each does with a full glass of water
 Do not increase or omit the dose
 Complete the entire course of treatment
 Follow directions
 Do not break, chew, or crush extended-release meds
 Avoid exposure to sunlight or ultraviolet light
 Notify physician if condition gets worse

Sulfadiazine: This drug should be administered at least 1 hour before or 2 hours after a meal

Uses- UTIs, chancroid, acute otitis media, Heamophilus influenza and meningococcal meningitis, rheumatic
fever

Adverse reactions- Vomiting, headache, diarrhea, chills, fever, anorexia, crystalluria, stomatits, uritcaria,
pruritus, hematologic changes, Stevens Johnson syndrome, nausea

Sulfasalazine: Trade name (Azulfidine, Azulfidine)

Uses- UTI, acute otitis media, Heamophilus influenza, meningococcal meningitis

Adverse reactions- Same as Sulfadiazine, may cause skin and urine to turn orange-yellow

Trimethoprim (TMP): Trade name (Bactrim, Bactrim DS, Septra, Septra DS)

 Renal disease

 GI disease

 Lactation

Effect of interaction: Decreased effectiveness of contraceptive agent with ampicillin an penicillin V

Tetracyclines

Common use: Anti-infective

Effect of interaction: Decreased effectiveness of penicillins

Anticoagulants

Common use: Prevent blood clots
