Professor and class,
The topic this week is, “Delirium: Why Are Nurses Confused?” The study I chose was
performed by Dahlke and Phinney. This qualitative study describes the challenges and
barriers that we as nurses have with caring for elderly hospitalized patients that suffer
from delirium. Twelve nurses were interviewed about their personal and clinical
experience when it came to the assessment, recognition and treatment for delirium.
After the interviews were examined, three separate concerns were found. The first
concern was “taking a quick look.” This is an instance in which a nurse performs a very
short, non-detailed assessment due to limited time. Keeping an eye on them is the
second concern. This consists of nurses making frequent rounds on patients who are at
risk for delirium. The third and final concern is controlling the situation which focuses on
the nurses intervening when necessary to provide care and prevent any injury to the
patient or staff. The study also put great emphasis on the importance of increasing
education related to delirium for nurses.
This research was interesting to me because the nurses were so truthful about the lack
of education, they have received in regards to delirium. It is such a misconception that
nurses are educated in every aspect, but that just simply isn’t the case. Most often we
as nurses build out knowledge based on incident or trial and error with patients who
experience delirium. As nurses we are only as knowledgeable as what we are provided
with and with the increase in delirium within hospitals it is important to provide the
proper education and training to nurses so that we can safely and efficiently care for
these patients. “Among hospitalized older adults, delirium is alarmingly common, with
30% to 40% experiencing an episode. The risk is even higher for patients who undergo
surgery or are admitted to the ICU ()”. The research in my chosen study was reviewing
how well nurses understood delirium, as well as how they felt about their current level of
knowledge on delirium. Both authors interviewed nurses to support their research and
determine the current skills the nurses had in assessing and dealing with patients that
had delirium. By questioning the nurses, the authors were provided with the information
they needed to make recommendations about nurse’s knowledge of delirium.
