DOMAIN: HISTORY
1a) Does anyone in your family have a history of a bleeding or clotting disorder? Besides
rest, have you tried anything to help improve the pain?

1c) The first question could revel family history of genetic bleeding or clotting disorders.
The second question could reveal any over the counter medications or other means of
pain relief the patient has taken.

DOMAIN: PHYSICAL EXAM
For each system examined in this case;
2a) Cardiovascular – This system needs to be examined because of the swelling in the
lower extremities. Swelling in the lower extremities can be inductive of heart problem.
Respiratory – This system needs to be evaluated in this case because a DVT is suspected.
Blood clots can travel to the lungs causing difficulty breathing that can lead to death.
Lower Extremities – This needs to be evaluated because this is the location of the chief
complaint. Both sides need to be compared to get an accurate evaluation.
2b) Cardiovascular – Abnormal findings could include a heart rate lower than 60 beats
per minute or greater than 100 beats per minute, an irregular rhythm, alterations in
carotid pulses, a murmur, bruit, thrill, or absence of S1 and S2 heart sounds ( Bickley &
Szilagyi, 2016).
Respiratory – Abnormal findings could include wheezing, crackles, rhonchi, barrel chest,
increase work of breathing, or use of accessory muscles (Bickley & Szilagyi, 2016).
Lower Extremities – Abnormal findings in this case includes the left leg being swollen
with pitting edema, the left calf being 3.5 cm larger than the right calf, and being tender
to the touch. The decreased sensation noted in this leg is also abnormal.
2c) Cardiovascular – Normal findings include heart rate between 60 and 100 beats per minute,
regular rhythm, absence of murmurs or other additional heart sounds, audible S1 and S2 heart
sounds, and blood pressure about 120/80 (Bickley & Szilagyi, 2016).
Respiratory – Normal findings include equal and clear lung sounds, respirations between 12 and
20, no use of accessory muscles, symmetrical chest expansion, a nontender chest wall, and an
absence of chest barreling (Bickley & Szilagyi, 2016).
Lower Extremities – Normal findings would be intact skin that is warm and dry, capillary refill
within 3 seconds, absence of edema, intact sensation, findings the same bilaterally, and full range
