Anatomie & fysiologie 2Plexus cervicobrachialis
Plexus= zenuwvlecht.
Tot ca. Th1. Vooral gevormd door de ramus ventrales. (na uittrede als ze de motorische en
sensorische wortel samen gekomen zijn gaan er weer twee uit elkaar. Dan ontstaat er een dorsale en
ventrale ramus.)
Plexus cervicalis (C1-C4):
- Sensorische en motorische innervatie van hoofd-hals regio
- Motorische innervatie van:
o M. sternocleidomastoïdeus (ook door n. accessorius XI (=hersenzenuw))
o M. trapezius (ook door n. acessorius XI)
o M. levator scapulae (ook door n. dorsalis scapulae)
o Mm. Scaleni
Plexus brachialis (C5-Th1)
- Vanuit elk niveau treedt er een zenuw. Dus er treden heel veel vezels uit.
- Uit ruggenmers, tussen scalenus medius en anterior door (achterste poort), onder de
clavicula, onder pectoralis minor en komt in okselregio terecht.
- Loopt tussen clavicula en de 1e rib.
- 3 trunci gevormd door:
o Superior (C5, C6)
o Medius (C7)
o Inferior (C8, Th1)
- Na opsplitsen en fusie (divisies) 3 fasciculi gegroepeerd rond de a. axillaris.
- Na splitsing in ventrale en dorsale takken groeperen rond a. axillaris
- Eerst truncus, dan fasciculi (lateraal, posterior, mediaal), dan zenuwen
o Lateralis: n. musculacutaneous (C5,6) en ook deel naar n. medianus (C6, .. T1).
o Posterior: n. radialis (C5..C8), n. axillaris (C5)
o Medialis: n. ulnaris en ook deel naar n. medianus.
Perifere zenuwen arm:
- Dorsalis scapulae
- Suprascapularis
- Thoracicus longus
- Musculocutaneus
- Etc.
Nn. subscapulares (C6-C8)
- Innerveert m. subscapularis (bovenste) en m. teres major (onderste)
n. thoracodorsalis (C6- C8)
- Latissimus dorsi
n. dorsalis sapulae: (C5)
- m. levator scapulae
- mm. Rhomboidei
n. suprascapularis (C4-C6)
- supraspinatus en infraspinatus
n. thoracisus longus (C5-C7)
- serratus anterior
n. muskulocutaneus (C5-C7)
- motorisch: ventrale bovenarmspieren (biceps, brachialis, coracobrachialis)
- sensorisch: lateraal onderarm t/m radiaal pols: n. (cutaneus) antebrachii lateralis
, n. axillaris (C5-C6)
- motorisch: deltoideus, teres minor
- sensorisch: klein gebied laterale zijde schouder
N. radialis (C5-C8)
- motorisch: m. triceps brachii, spieren dorsale zijde onderarm (mm. Extensor carpi radialis
longus en brevis, m. extensor digitorum, m. brachioradialis, etc.)
- sensorisch: huid (deel dorsale zijde onderarm en hand, ramus (Cutaneus) superficialis nervi
radialis.
N. ulnaris (C8-Th1)
- motorisch: m. flexor carpi ulnaris, m. flexor digitorum profundus (IV en V), mm. Pinkmuis, m.
adductor pollicis, m. flexor pollicis brevis (profundus), mm. Interossei, mm. lumbricalis III en
IV)
- sensorisch: huid (o.a. ulnaire zijde onderarm en deel hand, ramus (cutaneus) palmaris en
dorsalis nervi ulnaris.
- Bij uitval= klauwhand. Hij kan pink en ringvinger niet goed buigen.
N. medianus (C5-Th1)
- Motorisch: de rest van ventrale musculatuur onderarm, rest van musculatuur hand.
- Flexoren in de onderarm: m. flexor carpi radialis, m. palmaris longus, m. flexor digitorum
superficialis en profundus, m. flexor pollicis longus.
- Pronatoren onderarm: teres en quadratus.
- Verder: abductor pollicis brevis, opponens pollicis, flexor pollicis brevis, lumbricalis I en II
- Sensorisch: huid (o.a. palmaire zijde huid duimregio), ramus (cutaneus)palmaris nervi
medianus
Kennspieren:
- C1: m. rectus capitis anterior en lateralis nekflexie
- C2: M. obliquus capitis superior, m. sternocleidomastoïdeus rotatie hoofd
- C3: M. trapezius descendens, etc.
Sensibele innervatie= kerngebieden zijn geel gemaakt.
Dermatomen= innervatiegebieden van de wortels, van een bepaald segment.
Dat is wat anders dan de perifere zenuwen, vanwege de plexus.
Verloop en entrapment
Passage RM (kanaalstenose, het zenuwweefsel zelf komt al in de knel)
Treedt uit in foramen intervertebrale (probleem bijv. met discus hernia, wortelinklemming)
Plexus:
- Plek waar de plexus door de achterste scalenuspoort gaat
- Tussen clavicula en 1e rib door.
- Onder pectoralis minor
- TOS= thoracis outlet syndrome
De fasciculi zitten om de a. axillaris. Een bindweefselschacht omsluit de neurovasculaire bundel.
Passage n. suprascapularis:
- Inscisura scapulae, daar gaat hij doorheen, daar zit ook een ligament.
- Rond spina scapulae
Passage n. axillaris:
- In laterale okselpoort.
- De nervus loopt samen met de arterie om de hals van de humerus heen.
Passage n. radialis:
- In de onderste okselpoort, onder de teres major.
- Loopt tussen de twee koppen (mediaal en lateraal) van de triceps brachialis. Loopt hier over
de achterzijde van de humerus. Ook vaak betrokken bij een bovenarmfractuur.
- De ramus profundus gaat door de twee oorsprongskoppen van de m. supinator.
