A. Innovation Proposal

The patients and their families should receive exemplary physical, emotional, and

spiritual care, and our patients' safety should be our priority. With this concept, I noticed in the

hospital that I work at that the newly graduated nurses (new grads) had poor wound vacuum

therapy (wound vac) management which compromised patients' safety. Due to the COVID-19

pandemic since 2020, university nursing students were banned from having clinical experience

in all hospitals. This caused a tremendous negative effect on the new grads because they could

not have hands-on experience with all the bedside nursing skills.

The new grads program of our hospital consists of 18 weeks of orientation. The first four

weeks consist of academic and stimulated classes where the new grads practice on a simulated

manikin.

A1. Role of Innovative Nurse Leader

Nurse innovators cultivate and advocate for innovative ways to improve healthcare

services, reduce healthcare costs, and enhance efficiency (Cianelli, et al., 2016). Nurse

innovators work with registered nurses, multidisciplinary team members, colleges of medicine

and nursing, healthcare institutions, and other disciplines. Nurse innovators also collaborate with

other healthcare organizations and professionals to promote quality health care services.

A2. Summary of Community of Practice

Due to the tropical weather of Florida, the rate of senior citizens who are 65 years and

over was increasing. There were 4.5 million retirees in Florida as of 2019, the second-highest

senior population number after California (Statista, 2020). The population in Palm Beach County

(PBC) was 1,496,770 as of 2021. 24.4% were 65 and older, 19% were below 18 years of age,

,53.5% Non-Hispanic White, 18.7% Non-Hispanic Black, 23.4% Hispanic, 2.9% Asian, 0.1%

Native Hawaiian/Other Pacific Islander, 0.6% American Indians & Alaska Native, and 51.5%

were females. However, there was an area of concern with the increase of 1% of children in

poverty in PBC (total of 16%) between 2007 and 2018. Although the uninsured rate in PBC

(18%) was trending down, it had always been higher than the other counties and national rates

(16%). Unfortunately, the education level in PBC (high school and college) was approximately

6% - 8% lower than the national average. Those who graduated from high school were 88% and

those who attended some colleges were only 65% (County Health Rankings & Roadmaps, n.d.).

Delray Medical Center (DMC) is a Tenet facility, which serves Palm Beach County in

Florida. The business headquarter is in Dallas, Texas. There are 65 Tenet hospitals in the United

States, 10 of which are in Florida (ScrapeHero, n.d.). DMC has 523 licensed beds, and it is a

level one Trauma Center and Comprehensive Stroke Center (Delray Medical Center, n.d.) It is

part of Palm Beach County’s Trauma System overseen by the Health Care District of Palm

Beach County. There is a heliport for the Health Care Districts Trauma helicopter, and it is used

for severe cases like motor vehicle accidents, a critical condition resulting from a severe fall, or a

violent altercation. Aside from board-certified emergency physicians, nurses who are certified in

advanced cardiac life support and pediatric advanced life support, an experienced team of

technicians, and a team of consultants are available when needed. Other services that DMC

offers are Bariatric, Cardiovascular, Orthopedics, Oncology, Neuroscience interventions, and an

acute inpatient rehabilitation unit.

Typically, from a nursing point, the employees report to an active director, whose chief

report is to the hospital's chief nursing officer (CNO). The CNO reports to the Chief Executive

Officer (CEO) (Delray Medical Center, n.d.).

, The patient’s safety issue identified was the new grad nurses' lack of proper wound vac

management. In this proposal, Communities of Practice (CoPs) are organized by people

(stakeholders) who are primarily interested in patients’ safety, increasing revenue to the hospital

by improving patients’ experience score (which is posted online), higher reimbursement (Nickles

et al., 2020), and focused on providing sufficient training for newly graduated nurses to build a

strong foundation for their nursing carrier. The mission of the hospital and the shared value of the

stakeholders are to provide high-quality medical care and patients’ safety. The stakeholders who

are involved are: CNO (who oversees the administration of patient care and nursing in the

hospital) is the project and budget sponsor. The nurse supervisor (who integrates hospital

management with clinical care) is the contact person between the Education department and the

team. The unit director (who supervises and manages medical professionals in their unit) will get

the list of the new grads in each unit. The charge nurse (who resolves conflicts between staff

members/patients and manages the schedule) will arrange the schedule of the new grads to be

able to attend the teaching session of the wound vac management. The floor nurses (who deliver

direct patients’ care) will prepare a presentation about the wound vac troubleshooting solutions.

The new grads program facilitator (who manages the setup of the academic topics of the

program) will reform the new grad syllabus and include wound vac teaching in every quarter

with Zoom link for live simulation lab training for the group that will not be able to attend in

person due to the pandemic safety precautions implemented by the hospital. My role as a

stakeholder is being the charge nurse (the innovator)
