I have been working at Home Care Agency for a few months now and the
Care Manager has asked me to attend multi-disciplinary meetings for two of
the individuals that I work with. The Care Manager wants to know how the
different professionals work together to meet the service users’ needs and the
roles and responsibilities of each member of the multi-agency and
multidisciplinary teams involved.
I am going to produce a report to explore how the Care Agency works
together with other organisations using my own role from the scenario and 2
other health or social care professionals who make up part of the
multidisciplinary team involved in the care of 2 individuals I work with.

Responsible for commissioning publicly funded social care services including
sexual health services, health visitors, domiciliary care services, residential
care services, school nursing and addiction services (What is commissioning
and how is it changing?, 2020). This will help Jim and R as organisations can
come together and help meet their needs.

Health and social care Ireland board was formed in april 2009. This
organization consists of five local commissioners and five health and social
care trusts. It has a role of reforming h&sc services so service users such as
R&Jim have better resources to help them and also to interpret provision in a
sufficient and economic manner.
Local authorities commission to help personalized , community based support
by promoting health and social support therefore allowing resources to
services such as Jim and R backed up by evidence and local knowledge so
they can help to the best of their needs and Jim and R needs are met. Local
authorities work closely with other organisations such as nhs and housing
using Adult social care outcome framework. For examples, these
organizations can work with Jim and R to see if they need any adaptations to
their health or living conditions so they are able to provide for them. This will
allow Jim and R to feel looked aftered and cared for.

The health and social care act 2012 initiated other boards in hera;lth and
wellbeing as a forum for local h&sc leaders. The point is to work corroborating
with other public services within h&sc. One of the many things they are tasked
with is assessing local needs within the area such as Jim and R. Assessing
Jim and R will benefit professionals as we can access any weaknesses within
any daily tasks Jim and R are faced with so we can gather resources for them
so it can be easier for them. Another one they are assessed with is setting out
any and sharing approaches for local authorities such as CCG and Nhs

, commission decisions so we can make the best decisions for jim and r. Lastly
another they asses with is collaborating together such as joint commissioning,
shared budgets so professionals have aq responsibilities to decide where the
money goes as its limited so Jim and R will have to met the criteria of a certain
item/help so they can come together to buy it for them which os why it's
important to assess. These boards were created in 2015 which aims to help
HWBS leaders gain support to work together more effectively. They have no
formal power so they rely on having great communications with organisations
and have relationships in order to make these changes to help Jim and R
happen successfully. They allow local leaders to work together and forge their
ideas and links.

Eligibility frameworks are key to help integrate health and social care. Their
help should be able to reduce inequalities in service provision e.g. R and Jim
should have the right of freedom to do what they like without professionals
getting in the way such as Jim going to church and R wearing feminine
clothes. They should receive greater clarity and consistency so professionals
should be consistent with Jim and R. One of things professionals should act
on is greater emphasis on the individual rather than the service so providing
proper help for Jim and R for their specific needs rather than general help
across all service users. Better integrations of services so they should work
closely together to provide for Jim and R. Reduced repetition of the person's
story so instead of repatong , provide professionals with only the important
information. Greater focus of prevention earlier on in the process . so helping
jim and R early and preventing dangerous acts before allowing any serious
damage that can lead to life long injuries.
An EHc plan is for children and young adults aged 25 and down who need
more support than is available through SEN ( special educational needs), it
can be used by parents, doctors , teachers and other authorities in the
person's life that may need it also they asses those who are in the plan.

Positive impacts agencies will have on services Users such as Jim and R ios
that they know they can depend on other people in order to help their needs
and that they are not alone leading to high self-esteem and worth. Providing
them these options will boost their confidence to ask for help and they will feel
included and involved in order to live a better life with the conditions they have
as a duty of care is being fulfilled. They will not feel embarrassed as they are
vulnerable but are being respected which will motivate them to want to help a
better life and to adapt to their new lifestyle despite the challenges they may
face. This can improve R mental health as well as he tends to feel depressed
