Blokboek – samenvatting
Artritis van een gewricht kenmerkt zich door zwelling, warmte en bewegingsbeperking (zowel actief
als passief). Roodheid wordt soms gezien bij acute kristalartritis of bacteriele septische artritis (hoeft
niet altijd). Bij een artritis zijn de pijn en zwelling te herleiden tot de gewrichtsholte en het kapsel.
Voor het diagnosticeren moet je als eerste onderscheid maken tussen periarticulaire problemen en
articulaire problemen. Ook moet je onderscheid maken gericht op de onderliggende oorzaak
(immuunziekte?). Peri-articulaire pijn is opwekbaar door lokale druk op het aangedane weefsel.
Articulaire pijn beperkt zich tot het gebied van het gewrichtskapsel. Bij articulaire aandoeningen treedt
de meeste pijn op in de stand waarbij de intracapsulaire ruimte het kleinst is: volledige extensie.
Inflammatoire aandoening; kenmerken van ontsteking: roodheid, zwelling, warmte, pijn en
functiebeperking. Roodheid wordt echter alleen in sommige gevallen van acute ontsteking gezien.
Artritis = ontsteking van een gewricht
- Monoartritis = ontsteking van 1 gewricht
- Oligoartritis = ontsteking van 2-4 gewrichten
- Polyartritis = ontsteking > 4 gewrichten
Artralgie = gewrichtspijn
Het onderscheid tussen artralgie en artritis is belangrijk voor de diagnose en kan alleen worden
gemaakt d.m.v. lichamelijk onderzoek.
