Five Open-Ended Questions with Rationales for HPI
One of the first questions that was asked during the visit with Buddy was his age.
Engaging a child in age appropriate conversation is important in gaining the child’s confidence
and decreasing their fear from the beginning of the visit (Bickley, 2017). Another important
question that is important to ask during the visit is “What brings you in today?” Since this is
Buddy’s first visit at the clinic, it is important to have a baseline of information regarding the
patient’s medical history, medication reconciliation and review and to address the patient and
parent’s concerns in order to build a trusting relationship. Buddy’s mother Jill was asked about
his immunizations being up to date. The rationale for this question is because vaccine-
preventable diseases have a costly impact, resulting in doctor’s visits, hospitalizations, and
premature deaths (CDC, 2018). Another important questions which was asked during the visit
was regarding any complications during Buddy’s birth or during his mother’s pregnancy. The
rationale for this is because the American Academy of Pediatrics recommends developmental
screenings during preventative health visits to identify any developmental delay and seek
intervention if needed (AAP, 2020). Lastly, a significant question to ask is regarding Buddy’s
family and family life. Buddy lives with his two mothers and recently relocated due to one of his
parent’s work. An annual health supervision visit provides the examiner an opportunity to assess
the child’s mental, physical, developmental, parent-child relationship and the child’s relationship
with peers and school performance (Bickley, 2017).
