D.P6 explain why meeting the need of the individuals requires the involvement of different
agencies

Introduction (P6)
Agencies work together to provide all the needs and support an individual need. Different agencies
work together and help improve outcomes for families and their children and providing them with
what they want. Without the assess to these agencies there will be a lack of coordination and people
will have to keep repeating their problems to different agencies and get no progress towards there
better. However, this is prevented by agencies communicating and working together and individuals
won't need to explain their problems multiple of times to different agencies as they will already have
the information. An example of agencies working together is in a school environment with a child
who has down syndrome. It is important that all the different agencies work together for example,
school staff, speech therapist and the child's parents will work together to help provide the child
with the best care possible. When they work together it will help improve their quality of work and
provides with better services and reduces the level of stress. (Sloper,2004). Another example of
agencies working together is for an elderly person like Aisha. The agencies would be carer, GP, their
family. They will work together and help ensure the elderly individual is provided with the best care
and information that is needed is passed on. This is so if the elderly has any food requirement as
Aisha is on a special diet because of religious reasons it is important she is provided with the correct
food. So, she can feel equal and not uncomfortable and discriminated because of their religion and
this will have an effect on their health. Another example is Billy, different agencies will work together
to ensure that billy is provided with a home and with the best care possible. The doctor can provide
Billy with the medication he needs and the optician can help provide him glasses to see how ever
these agencies will work together the help ensure that the reason his eye sights are poor are not
related to a different health condition. The doctors can help Billy by informing housing Assocation
about Billy health conditions and provide him with a house to live in. BEST intervention is an
organisation that may be involved while providing care. They are seen to have a lot of positively
impact on parents. They have good access to services through contact with the BEST and signposting
to other agencies. Another is extended schools offer services which helps to identify a positive
impact on Child's attainment and over all life chances. They involve smooth referral systems to multi
agency support and help improve staff recruitment and workload as well as working with others in
the community to provide a better school security.

Role of organisations responsible for commissioning health care-
clinical commissioning groups (CCGs) in England (P6)
The CCGs was formed by the NHS England on April 2013. The CCGs take a greater delegated
commissioning responsibilities for the GP services. They aim to provide patients, communities and
clinicians with more scope in deciding how the local service are or can be developed and gives them
more influence over the NHS budget. The roles of the CCGs are to assess all the health needs of the
area, and to commission hospitals and community NHS services in the area such as, rehabilitative
care, urgent and emergency care, hospital care and services for learning disability. The members
that are involved with this organisation is GP practices, and other different health professionals such
as nurses. The CCGs helps meet and promote individuals needs as they are making a positive
difference by giving individuals a chance on a healthier future, helping cities and economies become
healthier, and they lead local partnerships and commission diabetes care. The CCGs also ensure that
services are culturally sensitive this is for individual who have a different cultural back ground and
health services must meet their needs for example Aisha may follow a specific food diet because of
her culture and religion. So, it is important that health services provide her with food that is based
on her diet requirement so that she doesn’t feel uncomfortable and treated unfairly because of

discrimination. It also ensures that services are of sufficient quality, so individuals like Billy are
provided with the good quality of care during his stay at the community hospital. If needs are not
meet this can cause individuals to feel uncomfortable and stress this will have an impact on their
recovery and it will take a longer time.

Roles of organisations responsible for commissioning social care services-
local authorities (P6)
The local authority is accountable for the environment and housing people live in, play, work and all
the opportunities they receive for leisure and work. The local authority aims to adopt a person-
centred approach to put individuals at the centre of their offer and meet their individual needs. The
roles of the local authority are to give advice, support and information to individuals that they are
entitled to. Accessible ways of retrieving information are provided to individuals such as; GPs offices
which are for the needs of local people, websites are provided and leaflets are given out. Another
role of the local authorities is to arrange and independent advocate to those who struggle with
communication and this will help people and help them to communicate their views, wishes and
feelings. The members involved with the local authority are the public health outcomes, adult social
care outcomes frame work, housing and NHS partners and the making it real statements. These
members help meet individual needs by giving individuals their needs and giving the care and
support they need to achieve their need. They also adapt homes and provide simple aids in order to
meet individual's needs. Local authority identifies preventative services in the area that it is needed
in. The local authorities will benefit individuals like Billy as they will be able to provide him advice,
support and information which he is entitled to.

Roles of bodies responsible for integrating health and social care-
health and wellbeing boards(P6)
The health and wellbeing board (HWBs) was introduced by the health and social care act 2012. It is a
forum for local health and social care leaders. The health and wellbeing board aims to integrate
public services in the health and social care sector. They are set with task to meet individual needs
like; to encourage integration and do more partnership like integrated provision, joint
commissioning, shared budget and leaderships. Another task they are set with is to assess the local
population needs and set out strategies and approaches for CCGS, NHS and local authorities to
improve commission decisions. Another task they are set is to reduce the inequalities in provision.
The role is to make a joint strategic need assessment (JSNAs) which is the current and future health
and social care needs of the local authorities. The members part of this health and wellbeing board
are the local authority director for adult and children social services, director of the public health for
the local authority and the local health watch organization CCG. The HWBs address the needs of a
demographic and engage in developing and supporting individuals. Boards like this is the Centre of
the care and health improvement programmed (2015), it aims to help with the development of the
HWBs leadership and support the integration of services better. Leaders from the health and social
system make up these boards. The HWBs have no formal powers however they build strong lines of
communication and relationship to achieve success. Local leader also gets together so ideas can be
shared and forge a link between local services.

The HWB are not effectively supporting individuals like Billy to meet their individual needs because
the HWB has no funding or enforcement powers they have to rely on building strong relationships.
Another reason why the HWB may not be effective in supporting individuals like Billy as there is no
recommendation on the number of members that can be on the board. So therefore, if it is big, it
may be difficult to make decisions and there for individuals like Billy won't receive the help and
support, they need to improve their life. Also, If the board is too small views will not be heard or
acted on. This will affect individuals needs and they will not be met. HWBs have three different
