Thema 5: KindABCD bij kinderen
Verschil tussen kinderreanimatie en volwassenen.
Bij kinderen 15/2. Ventilatie is belangrijker dan circulatie.
Altijd een AED aansluiten want kleine kinderen hebben bijna altijd een
schokbaar ritme.
Tot 2 jaar met 2 duimen.
1. 5 beademingen
2. 112 bellen
3. AED aansluiten: een midden op de borst – een midden op de rug – shock
geven
4. Indien nodig reanimeren : 15/2
Bij kinderen 75 joule ipv 150 joule. Je mag ook volwassen plakkers en joule
gebruiken bij kinderen.
Kinder ALS
Gedrag: afwijkende tonus (heel slap), verminderd bewustzijn,
ontroostbaar, abnormale spraak
Ademhaling: bijgeluiden, intrekkingen, neusvleugelen, apneu/gasping
Kleur: bleek, gemarmerd, cyanose
Luchtweg bij kinderen
1. Trachea is korter, want kind is kleiner. Kraakbeenringen zijn nog heel slap.
Bij snel inademen zal trachea beter samentrekken.
2. Tong is relatief groot in een relatief kleine mond. Tong zakt sneller de keel
in.
3. Het nauwste gedeelte zit onder de stembanden (bij volwassen is het de
stembanden)
A - Mondinspectie, maar je zal niet heel veel zien.
- Bij huilen is de A vrij
- Nooit je vinger in de mond doen, die is in verhouding zo groot dat je een
evt corpus alienum alleen maar verder duwt.
B - Symmetrische thoraxexcursies
- Ademhalingsfrequentie
- Neusvleugelingen
- Gebruik hulpademhalingsspieren; lukt niet zo goed want die spieren zijn
nog niet zo goed ontwikkeld. Daarom zal je het hoofd zien mee knikken
omdat m. sternocleidomastoideus probeert aan te spannen = head
bobbing.
C - 5 sec op sternum drukken, CR >2 sec
- Pulsaties voelen: carotis voelen is lastig door kleine nek, brachialis en
femoralis is makkelijker.
- Harttonen
- Bloeddruk meten en hartfrequentie
D - Hersenen hebben veel glucose nodig. Bij even hypoglycemie kan al
schade geven.
- Eerst sluit achterste fontanel, daarna bovenop het hoofd tot 1 jaar.
- Pupillen: pearl
- Houding: decorticatie (armen gebogen, handen bij je cor, probleem in je
cortex), decerebratie (armen gestrekt, probleem in het cerebrum)
- Altijd zuurstof geven, maar teveel kan ook schadelijk zijn.
