Handboek H1: Inleiding
Een somatische aandoening dwingt het individu om hier continu rekening mee te houden.
Niet iedereen beschikt echter over het aanpassingsvermogen om hiermee om te gaan.
Wanneer dit te veel wordt, kan een psycholoog ingeschakeld worden. Lichamelijke en
psychische problemen komen vaak samen voor en kunnen elkaar ook negatief beïnvloeden.
Er is een onderscheid te maken tussen ziektegenerieke en ziektespecifieke aspecten.
Ziektegenerieke aspecten zijn lichamelijke, psychologische of sociale klachten die
voorkomen bij verschillende ziektes. Ziektespecifieke aspecten zijn klachten die voorkomen
bij een specifieke ziekte of voortkomen uit de fysieke gevolgen van de ziekte en/of medische
behandeling. Hoofdstuk 2 t/m 28 van het handboek richt zich op ziektegenerieke aspecten en
hoofdstuk 30 t/m 40 richt zich op ziektespecifieke aspecten.
Handboek H2: Medische psychologie
Gezondheidsproblemen worden vaak ervaren als bedreiging en maken mensen afhankelijk
van artsen en andere professionals. Dit zorgt ervoor dat mensen met lichamelijke klachten
kwetsbaar zijn. De reactie van de patiënt heeft vaak gevolgen voor het beloop en de ernst
van somatische aandoeningen en het effect van medische behandelingen. Een derde van de
patiënten chronische of ernstige somatische aandoeningen ervaart daarnaast psychische
problemen als gevolg van de ziekte.
Een medisch psycholoog wordt ingeschakeld wanneer de basiskennis over de psychologie
waarover de arts beschikt onvoldoende is om de patiënt te helpen met hun psychische
problemen. Een medisch psycholoog is een BIG-geregistreerde GZ-psycholoog, klinisch
psycholoog of klinisch neuropsycholoog die werkzaam is in een ziekenhuis.
Een medisch psycholoog kan op verschillende manieren ingezet worden. Allereerst kan de
psychologische patiëntenzorg indirect zijn en bestaan uit advies tijdens een
multidisciplinair overleg (MDO). Tijdens een MDO kan een psycholoog ook achterhalen of
er sprake is van ziektegenerieke of ziektespecifieke problematiek. Ziektespecifieke
problematiek wordt behandeld door een medisch psycholoog en ziektegenerieke
problematiek kan behandeld worden binnen de reguliere GGZ.
Hiernaast kan een medisch psycholoog direct in contact komen met de patiënt tijdens een
klinisch consult, poliklinische intake, diagnostisch onderzoek en eventueel een
psychologische behandeling. Een klinisch consult kan plaatsvinden wanneer een patiënt is
opgenomen in het ziekenhuis. Een klinisch consult is geïndiceerd wanneer de medische
diagnostiek en behandeling stagneert door het psychisch of sociaal functioneren van de
patiënt of wanneer de communicatie tussen patiënt en arts ernstig verstoord is. Achteraf
bespreekt de psycholoog de probleemanalyse en het advies met de arts, waardoor deze een
nieuw perspectief kan vormen op het functioneren van de patiënt. Mogelijk wordt het
medische behandelbeleid aangepast. Een eventuele kortdurende psychologische behandeling
bestaat vaak uit psycho-educatie en cognitieve gedragstherapie.
Wanneer een patiënt niet is opgenomen in het ziekenhuis kan een poliklinische intake
geïndiceerd zijn. Hierbij wordt eerst beoordeeld welke ziektespecifieke psychologische
expertise nodig is en wie er wordt uitgenodigd voor het gesprek (alleen de patiënt of ook
andere betrokkenen). Naast het gesprek kunnen ook vragenlijsten ingezet worden om meer
informatie over de patiënt te verkrijgen.
