1. klinisch meest relevant is de achteruitgang van de nierfunctie,
gemeten door de glomerulaire filtratiesnelheid (GFR) in het
bloed, dit neemt 1 punt/jaar af
a. Gemiddelde snelheid bij 70+ is 60ml/min
b. Bij kwetsbare oudere pt met multiple comobriditeit en
polyfarmacie is dit 40ml/min
c. Dit kan worden beïnvloed door intercurrente zaken bv. dehydratatie,
medicatie (NSAID, ACE-inhibitoren) en onderzoeken met IV-contrast
d. GFR-waarden zijn niet betrouwbaar bij mensen met acute
nierinsufficiëntie, pt met verminderde spiermassa (dus let op bij sterk
vermagerde pt en sarcopenie), langdurig bedlegerigen, ernstige
ondervoeding of met verlammingen
e. als de GFR daalt dan worden afbraakstoffen, ook geneesmiddelen
minder snel uit het lichaam verwijderd, hierdoor worden ze als actieve
