HC Infecties bijtransplantatie
Bij een transplantatie worden 3 middelen gegeven ter voorkoming van afstoting:
1. Prednisolon (cortico)
1. Remt de vorming van Nucleair Factor kappa B, wat nodig is om het DNA af te lezen
voor pro-inflammatoire moleculen, waardoor je zowel B-cellen als T-cellen remming
krijgt.
2. Calcineurine remmer (ciclosporine en tacrolimus)
1. Remmen de calcineurine pathway. Er zal dat geen T-cel proliferatie zijn.
3. Nucleotide analogen (mycophenolaat (MMT) en azathioprine)
1. Remmen de purine synthese waardoor geen DNA replicatie kan optreden. Hierdoor
krijg je met name geen T-cel respons.
Casus
VG/ Autosomale dominate polycysteuze nieren, recidiverende UWI, hypertensie.
Niertransplantatie 2001.
Med/ linisopril (ACE remmer), hydrochloorthiazide (diureticum), prednison/MMF/TAC
A/ pt wil AB want blaasontsteking. Sinds drie dagen branderig gevoel bij plassen,
kleine beetjes en pijn rechts in de zij. 1x bloed gezien. Voelt zich niet lekker.
LO/ niet ziek, temp 37,9, bloeddruk 90/65 en HF 77, flankpijn rechts, transplantaat niet
drukpijnlijk, blaasdemping onder symfysis pubis, cystenieren zijn te voelen.
DD/ prerenale nierinsufficiëntie door 1. Infectie nier 2. Misschien NSAID gebruik? 3.
uitdroging
Lab/ Creatinine 166 umol/l (95 umol/l normaliter)
Urine sediment : 150 ery’s per microliter (bij herhaling) veel leukocyten ACR < 3 mg/g
of ECR <150 mg/mmol (geen proteinurie)
Echografie transplantaat: geen hydronefrose en een lege blaas.
B/ stimuleren intake en stoppen ACE remmer/diurecticum. AB starten (ciprofloxacine)
Voorgaande urinekweek: E. coli gevoelig voor ciprofloxacine. Urinekweek afnemen
voor start antibiotica. Poli controle met bloedonderzoek in 5 a 7 dagen. Stop de
immuunsuppressie niet!

- 50% van alle infectieuze complicaties
- Incidentie tot 84% in eerste en 30% in tweede jaar
- 10% presentatie met urosepsis
- Recidiefkans 40%
